I don't think it would be nearly that awkward. I think Addison would go into doctor mode and focus on that, and Meredith and Derek would be too focused on her and the baby. Even at the beginning they didn't actually hate each other, and now many years have passed. I'm pretty sure Addison is remarried by now too with a kid. Plus having a medical situation would be a distraction and would make it less awkward.
