Oh well, Richard, I just failed my test; my examiner said that the major is I did not check the blind spot before moving off. I do think I have checked, but he said, I need to look over my shoulder every time....
I'm sorry to hear that, usually failing to check the blind spot when moving away is a minor mark. But if you do this throughout the test or there is something there, then it's likely a fail. Try and keep up the practise, I wish you the best of luck for next time.

Signal just before you're about to do something. So if you're turning left, signal just before you start to slow down. If you don't need to slowly down try and give people adequate warning without the signal being on so long that it confuses people.
A cyclist could have been riding up the inside to be clear of traffic on the outside of you. Don’t assume all cyclists will go around. Not looking in that blind spot on N/S is (in my opinion) wrong and I wouldn’t leave it out.
When pulling away from the right do you need to signal if there are oncoming cars like you would if you were pulling away from the left? And do you leave the signal off if there are cars coming behind you as you would if you were pulling away from the left?
When pulling away from the right you wait until you will affect no cars so a signal is not needed for cars on the road. But a signal may still be needed for pedestrians or anyone else on the pavement who may be planning to enter the road.
You will need to check which ever mirrors give you a good view of behind. Generally the centre and right mirrors will give you the best view when parked on the left side of the road.

Richard, make some mock tests like World Driving with a odometer/gear change on the screen/speed limit signal on screen.
I have already made a mock test video. Will be available before the end of the year if I remember correctly.

I think for an emergency stop there is no need to check centre mirror for behind

Should I set the gas before checking the mirrors and blind spot then find the bite point
Yes, that's what I recommend beginners do because the gas can take some time when you're not used to it.

What about if you’re stopped at a round about do you still need to check mirrors
If you're waiting to turn left at a roundabout I would be aware of my left mirror for cyclists. But my main focus would be on the roundabout itself.

If you’re parked on the left hand side of the road which mirrors do you look in before you move off
