  • Thank you for watching our ear wax removal video today. The ear wax extraction featured in todays video was carried out using endoscopic microsuction. This procedure uses a small camera called an endoscope to visualise inside the ear canal whilst a small suction tube is inserted to gently remove ear wax.

    Ear wax plays a vital role in ear health. It helps to prevent ear infection and helps with ear cleaning by picking up any dirt or debris that accumulates in the ear canal. Normally this ear wax would fall out of the ear taking any dirt or debris with it.
    How to remove ear wax?
    Many people use q-tips/cotton buds for ear wax removal at home this is one of the worst things you can put into the ear as this can lead to an increased ear wax buildup or in extreme cases impacted ear wax. The best way to remove ear wax is to let your ears clean themselves and not to put anything smaller than your elbow in your ears!
    How do I know if I have an earwax blockage?
    If you experience a change in your hearing, discomfort in the ear or jaw or a feeling of fullness in the ear canal you may have an ear wax blockage. Our advice is to speak to your Doctor or local Audiologist who can take a look in your ears and assess whether ear cleaning is needed.

    It must be similar to the first time you put on a new pair of glasses. I was maybe nine years old when I first got glasses, and I still remember how amazing it was to be able to see individual leaves on trees, and how mind-blowing it was that everybody else could see so much more than what seemed normal to me.

    Patient is right about microsuction in the United States. I didn't even know existed until I started watching your videos and realized that I needed it. Irrigation was just not working and I was having a hard time hearing. I was lucky because assistant in ENT department just started working a new how to use micro section two clear out my ears.

      I have had hearing problems my entire life, in fact my husband thinks I just don't pay attention. But if I can see the person's face especially in areas with background sound I don't hear them very well. I can only imagine how clogged my ears are and they are constantly itchy. I would love to get this done and see how much better my hearing gets

      I wish we had this here in Texas. My husband has small ear canals that make loads of wax. If we ever roll out this option in the U.S. I’ll sign up asap to train and provide this service. We need it so badly here.

    This cleaning had to be gratifying on so many levels. You had someone come from the US trek from England to Wales just to see you. You successfully cleaned a very impacted ear. And you changed someone’s life. Not bad for one day. And, Rhys, even though Taylor is now the star, we will always want you around as eye candy. Mrs. B knows what I mean. 🙂

    My husband was complaining his right ear felt blocked and couldn’t hear as well even with his hearing aids. From watching your channel, I knew he had wax buildup. He went to an ENT Dr and he suctioned out half of the wax. Remainder wax he could not get so he had him do drops for 5 days and return. Dr was able to completely clear both ears. All with suction. A Family practitioner will only do irrigation. I’m in California

    I'm a US pediatrician. I do not know any audiologists who do wax removal. If they have a patient with wax, they have an ENT remove the wax. Of course, as a pediatrician, the audiologists I use are all co-located with ENTs.
    Although ENTs use endoscopy in their practices all the time, the ones I've worked with use an otoscope with an operating head or an ENT microscope for cerumen removal.
    I have not been able to find out if cerumen removal is in the scope of practice of AudDs in my state. (AudD is Doctor of Audiology... the only degree in audiology currently offered in the US... is considered a "professional doctorate" level degree. You do a 4-year bachelor's degree in anything -- often communication disorders or related field, then a 4-year audiology degree. Yes... they do 8 years of school and still aren't qualified in cerumen removal in some states).
    I know that there are audiologists in some states who do wax removal (because they're on CZcams) but they primarily use irrigation with some use of manual tools. They do not use suction, nor do they use endoscopes.
    To make things just THAT much more confusing, our licensing regulations vary from state to state, so the scope of practice in one state might be massively different from the state right next to you.

      I’ve had blockages removed at the walk in, I think there are only some people who can do it there though. They just squirt warm water and hydrogen peroxide in your ear and let everything run out into a cup

      Hello. U.S. audiologists do have cerumen management within their scope of practice, however it often more of a specialty, much like how not all Au.D.s work with CIs or vestibular testing.
      Part of the challenge is that compensation for this service are quite low, and if the service is provided my an audiologist due to necessity to perform other hearing tasks, such as an evaluation or hearing aid service, then the cerumen removal will not be paid by insurance. If the ENT does it, however, the procedure is paid out by insurance. Crazy, I know.

    Your patient is correct. I live in the U.S. wear a cochlear implant in my right ear and a hearing aid in my left ear and my ENT uses microsuction to clean my ears. Audiologist do not do ear cleanings (at least mine does not) and if your primary care physician (general practitioner) uses irrigation only. Irrigation always makes things worse in my ears.

    Who knew that audiology associates would become a vacation destination! 🤩 yes here in the US they send people home to “self irrigate” a mix of warm water and DISH SOAP. I cannot imagine the number of injuries that result from that insane suggestion!

      OMG, that's horrible. I am a registered nurse in Canada. Doctors do not use dish soap for ear irrigation. Only warm sterile water or olive oil to soften the earwax before removal. It's dangerous to put a dish soap inside the ears. The ex- president of the US, told the public to drink Javex to kill Covud-19. 😂😂

      Don’t make generalizations. My brother has walked into a clinic and gotten chunks of wax removed by a primary physician. Just depends on where you’re at in the US and the doctors.

    I’m also a parent who has resorted to tweezers to clear my son’s ear. The pediatrician wouldn’t do anything and by the time he was in first grade both ears were extremely infected and he was deaf. I had to take him to the Navy hospital to get antibiotics for him. After the course of antibiotics, I would have to weekly use peroxide and then tweezers to pull out massive plugs. He had to be held back in first grade because of not hearing the teacher the entire school year.

      I use ear candles on my son. He had repetitive sinus infections that would end up as ear infections. He went to an ENT and had persistent fluid so we did Zyrtec for a while. Not that he’s older he just tells me when it bothers him and we do the candles. They are easy to get

    Indeed, we do not have access to microsuction in the states. I had constant swimmers ear as a kid and always had my ears irrigated. By watching you guys I learned that the treatment was adding to the problem!

      We absolutely have access to Micro suction, and it’s done by ENTs. Lavage(irrigation) is done mainly in primary care,ER,Urgent Care.

      I am 75 years old and I live in Charlotte, North Carolina. When I was a child my mother noticed I wasn’t hearing well and took me to an ear doctor. The doctor suctioned out my ears. I remember the sound it made was very uncomfortable because it was in my ear! But I could hear normally after the procedure.

    In the US the ENT can do suction. It works so well but the insurance companies want control so they will only pay at the ENTs which is ridiculous. Great job you both do!

    Wow, the timing is uncanny! I’m a 60 year old disabled widow and have been having so many problems with my ear lately and have been unable to find someone to help me out. I’m also in the USA in Delaware and I’m a huge fan of this page for a few years Now. I’ve recently checked into the cost for flights and considered seriously coming to the UK to visit Your clinic for help with my problem. It’s very hard to find an ENT or specialist here where I live.

    He spoke the truth about America. ONLY an ENT does suction. But on the flip side I don't understand why none of his doctors referred him to one? Maybe his insurance didn't cover it. I know that most insurances won't cover hearing aids, even for a baby or child because of the massive cost on the other end when people get older.

      Somebody in another thread mentioned how licensing requirements are often vastly different across the states; the other thing which you kind of get at is how truly baroque insurance coverage can be. Different plans cover different kinds of service to varying degrees. Some require referrals to specialists; others do not but may have their own restrictions.
      In most non-trivial cases, it is literally impossible to figure out how much you will pay for any given treatment. It’s possible to go to an in-network hospital and be treated by an out-of-network (read: “we won’t cover this”) doctor. Particularly in an emergency, you might get bitten by even more bizarre cases, like a covered surgery and an out-of-network anaesthesiologist. That and it’s all tied to employment, so if you get a new job in the same city you might have to find a new primary care physician.
      I’ve lived here my whole life and I barely understand how it works, and I feel like for Rhys and Taylor it would be like trying to explain filmmaking to a pigeon, just in terms of the base concepts being completely foreign, figuratively and literally. (This is, of course, not at all a knock on our gracious hosts. Pigeons are lovely birds, and it may be possible to get one to understand the difference between copay and coinsurance, a distinction that is beyond me.)

      @@PaulFisher That is one of the best sum ups of insurance problems in the US that I’ve ever read. It is bizarre to me that I can go to an in network (read covered) emergency room but be treated by an out of network (read NOT covered) doctor with NO OTHER DOCTOR available. AND to not be told prior to treatment that you are screwed. You know that they ran the insurance before you’re even admitted so they know the doctor is out of network. 🤬

      @@denisemurphy3915 yes. Also, people need to understand that cochlear implementation is rarely a cure all. It usually works best if their is some history of hearing or other factors. My daughter was born deaf and because I live in Oregon and my husband's job had put everyone on leave for 6 mos for a refit I was on. The Oregon health plan for her birth. It then automatically covers the 1st year of her life. That is when we learned that his work insurance would not have covered the aids.

      Right? Two hours each way to go get an important procedure like this done, even if you’re the type that needs it every six months, seems entirely reasonable to us! But in the UK 2 hours is a *long* trip lol. I rode trains around the countryside there and the trips felt so short and nice, far more pleasant than any car or plane.

      Typical problem the US has. They find one way of doing something which is not particularly optimal but they get very attached to it and take ages to to change it.
      Another example is 4 way stop lines. It's a terrible junction and very inefficient in terms of traffic flow. You won't see 4 way stop in UK. Roundabouts or traffic signals have much higher capacity.
      And yet I saw a survey asking Americans what was better junction a four way stop, traffic signals, or a roundabout/circle.
      Most of them said 4 way stop was best. They know what they like and they like what they know. Quite an insular country.

    Irrigation is really the prevalent treatment for earwax buildup in the US.
    I don’t think there’s even a practitioner that does microsuction in the Seattle area…and that’s a big city!

      There are some providers in Seattle who do microsuction, but not many. Mill Creek Hearing and Balance used to have someone fantastic however he retired recently. UWMC, the VA, and most ENT offices do have it. Proliance ENT has it at a number of their locations. Pretty sure Evergreen Sppech and Hearing has it as well.

    As an expat, 2 hours is not a long journey in the US. I live in New York state, but New York City is a good 6-7 hours' drive from here (twice the time it takes to drive from Cardiff to London). Also, I have no idea what you charge, but I suspect the co-pay with insurance for this in the US would probably be higher than what you charge.

    A GP from across the pond here. I enjoy you guys, your banter, and your professionalism and dedication to your patients. You are correct, in the US we do irrigation at GP offices, to get microsuction a patient has to see ENT. Audiologists are not allowed to do what you do in the US, neither irrigation nor microsuction

    Almost all the audiologist use irrigation in the US. In California, I have only had suction at the ENT's office.

    I love that they're amazed at a 2 hour trip to go see them. I heard it and wasn't even phased. When my parents got divorced my dad would drive out an hour to pick us up and we'd be in the car for another hour to get back to his house on every Wednesday and every other weekend. To have my ears done professionally 2 hours is a drop in the bucket.

    (USA here) I ordered medical grade olive oil because of you too. I have excessive skin in my ears, and finally bucked & ordered it since i can't from you all. Helps with the itchy feeling on the inside. Hope it'll help the skin migrate better here on out!

    I must be very lucky in my area because I found an ENT right in the very hospital complex where I work that performed microsuction on me. It relieved my itchy ear immediately. I wouldn't mind a holiday in the UK to visit you wonderful gentleman though! 😊

    It depends on the audiologist. I always use tools suction, ear hooks, curettes, forceps. I only irrigate as a last resort or to help soften the wax. It depends on the setting, too. I did not have an ENT in my setting. I am in the USA

    I am not ashamed to admit I have frequently considered traveling from the US to Wales just to get an appointment with y'all.

      I am an audiologist, used to work in private practice, and I do suction. Within my field I am considered to have a specialty.
      I lived in one of the most expensive cities in the US.
      Bilateral cerumen management was be paid out of pocket for $25.
      If I was testing the patient or doing other hearing aid related services, then the service was free.
      Doctors of Audiology make, on average, 88K per year across the US. Newer audiologists' form of splurging is a coffee out once a week and paying student loans. Sometimes we dream of a nice dinner out.

    In 2007, I ended up with a bacterial and fungal infection in my right ear from swimming in the apartment complex's pool. I went to my primary doctor who used irrigation. That temporarily restored the hearing, but a couple of days later I was deaf in that ear. Finally, I went to an ENT who painted my ear purple by using gentian violet. The ENT also performed micro suction. I remember that he said things to me like don't grab my arm during the procedure. I guess previous patients had done that to him. Also, the co-pay was $50 a visit, and it took 3 visits to address the swimmer's ear infection. He also prescribed Dermotic Oil which is a steroid that helps to restore moisture to the ear. That cost $50 for a tiny bottle. When you remove all the wax from your ear, your ear lacks moisture and is really raw and tender.

    I’m in the US and have never had irrigation wax removal. I’ve always had micro suction done. I live in FL and grew up in VA. I always had it done by ENT’s.

    My son had to go to an ENT and they did micro suction. When the doctor pulled out the suction my son told the doctor that I’m always looking at these videos. Also audiologist here are doctors who deal primarily with hearing issues.

    They use some kind of loosening drops and then washing it out with a syringe. I have never heard of anyone getting the type of treatment you all do with such care and precision. From a faithful USA viewer.

    That was so satisfying to watch. I could almost feel the patient's relief once all the really big chunks of wax was removed. Poor guy he must have been feeling 100X better flying home especially when his ears were popping on the flight.

    As someone in the US. I wanted to say thank you for what you do on your channel. I didn't know about Audiologists before watching your videos. After watching them, I looked for and was able to find an Audiologist in my state of Arizona who has taken care of multiple of my family members ear wax issues. Since then, they have had such a quality of life improvement because of it. Idk if you'll read this, but just know that you posting your videos has helped my family and I am thankful for that!     I had a friend (USA) whose son had tubes put in his ears and was still getting infections. They removed his tonsils and the infections stopped. Tubes in the ears are very common in FL.

    As an American, I didn’t know microsuction was a thing before finding you lads. I wouldn’t even know what kind of clinic to go to here for wax removal.

      But yes, a procedure like this would take several steps to set up and probably a few months’ wait. Because for most insurance to cover care you have to visit your GP for a referral to a specialist. Getting the GP appointment alone can take over a month. Then you have to wait for your insurance and the specialist’s office to process the referral. THEN you have to book with the specialist and for new patients that can be a two-month wait. And THEN the doctor might not have time for an office procedure on the same day as the examination, so you have to book ANOTHER appointment ANOTHER several weeks out. And you get billed for every single visit.
      As a final twist of the knife, SOME procedures and medications require “preauthorization,” meaning the doctor has to contact the insurance and justify why the patient needs the treatment. And at that point the insurance can still just outright deny coverage. Oh and there’s still going to be a bill for the treatment itself separate from the appointment bill.
      If I had wax problems and were starting from scratch (assuming I would need to go to an ENT), I could easily end up paying $150 or more to go through all of these steps and get microsuction. On top of waiting months and fighting with insurance that will do everything in their power to NOT pay for my care.
      It’s truly agonizing.

    So true in US need ENT consult to get irrigation, never heard any micro suction. Retired (40 yr) nurse speaking from experience. Also have to wait probably 3 to 6 months for appointment. Cheers from north Florida

    It’s basically the same here in Australia, I had a blocked ear first time ever and went to my Dr where they performed a procedure of using water to flush out what they could but couldn’t remove it all, I then had to see an ENT specialist to remove the rest of the wax in my ear, but I didn’t have any where the amount of wax that you guys share on your channel but just enough to give me ear ache and when that small bit of wax was removed, I could hear perfectly.     I remember being absolutely fascinated and confused the first time I saw ear wax removal videos like this. I've struggled with overly waxy ears my whole life and have to get blockages cleared out every 3 or 4 years, and all they've ever done for me is blast a bunch of water/peroxide into my ear canal. The last time the doctor also scraped around with some tool to try and get some of it out and was so rough I ended up having bloody scabs coming out of that ear and then an ear infection for a few days after it. I would love more than anything to have a specialist who's as gentle and careful as either of you two are.

    If you have insurance, making an appointment in the US is easy. Surviving the extensive wait until the day finally arrives... not so much. Clinics are randomly cancelling appointments at a higher rate than ever before, and only 10% of those cancelled appointments are immediately rescheduled. Most of the time, the patient has to initiate the process all over again. Madness.
    I, too, never knew microsuction or even the use of oil and specialized tools were options until I saw your videos. Irrigation is the main go-to treatment for sure.

    The lesson of this video is however imperfect our NHS is we must do all we can to make sure that it remains free at the point of need. I've worked in the Netherlands and had all my healthcare fully paid for and I've worked in the USA and had pretty good health insurance but it was eye opening what wasn't covered by my insurance.

    Me in the US having constant ear infections and unrelated skin issue and I asked for a dermatologist and my gp who answers one email one day a week decided she was a dermatologist for the week and misdiagnosed me. Didn’t get me my referral just gave me some random ointment. Here it’s so hard to get care without jumping through a million hoops.

    American here, and it may depend on where he lives but I know where I am my pcp can do irrigation, but the micro suction can only be done by an ENT and depending on if you’re in a rural area it could be difficult getting in. I’m in a large city so I’ve never had any issues getting into a specialist. Either way I’m sure this patient feels sooo much better.

    Heartfelt thanks for helping this man from the USA. I am also. Never heard of what you do until I saw your channel. You have to go to an ENT dr in Iowa for your ears but mind does not do suction and my insurance refused to pay for it. Just seems crazy. All they do were I live is irrigation and that I did not like. My health will not permit me to travel or would come and see you both.
    So interesting that America and the UK is so different on this kind of ear work. Maybe your show will influence them to allow it here????

    In America it is much easier to find curitage like at a hearing aid center. We actually bought a kit on Amazon to save money. It works with a camera that shows on our iPhone. Seems less efficient, though,than your microsuction. Well, maybe our medical system here will grow up soon.

    Yes, in the US, if you have any ear issues, your primary doctor will refer you to an ENT. I know they use the water irrigation method. Luckily, I have very little ear wax. My doctor always says my ears are clear. It always blows me away when I see that much wax coming out of someone's ear!! No wonder they can't hear!! Thank you for helping these people.

    I'm in the US, in Wisconsin. I recently had an appointment with my ENT. At the end of my appointment she had her nurse clean my ears. She used microsuction and curettage.

    FWIW, I work in an internal medicine teaching clinic. At this level, if a patient came in with impacted cerumen, we would use a hand spray bottle with an irrigation tip that fits into the canal, using a warm water and peroxide solution. Our clinic, and to the best of my knowledge, none of our clinics have suction. No idea what our ENT/Audiology clinic has/uses.

    in Texas my husband went to the the Audiologist and I had the suction. His ears were clear, but needed hearing aids anyway. it was $95. for the visit and the hearing aids were $2000.

    It’s the same in Canada, we can’t just walk into an office to have our ears cleaned. Either through irrigation or micro suction. I have had an acute mastoid abscess removal and it was your channel that educated me on my ear canal shape as I had never been informed of this up to this point. But I still can’t get specialized ear cleaning. 😢

    Ok, I can't stand it anymore. I have refrained from making a comment about this, but here it is:
    I don't understand why the patient from America, and for that matter, you two and other patients, could not get an appointment with someone who does microsuction. I was at my allergist office, and we were talking about my ears having problems, and he recommended I get them cleaned. I was so excited when the technician said she would be using a microsuction machine. I was jumping for joy and told her about y'all and your channel. She could not believe there would be a channel dedicated to cleaning ears. I guess in Houston, TX, we are more medically advanced than than the rest of the country. Allergy & ENT Associates are who I go to. They have offices all over town.

    It’s a lot of our insurance issues! They’re specialist so a lot of insurance companies require we get a referral and then we have to make sure the office takes the insurance. It’s a mess

    Usa and Canada are similar, in the fact that your GP will pretty much do irrigation for you, but if anything else is deemed needed, you pretty much have to get a specialist referral. At least in Canada, specialists are generally covered by our universal health insurance.

    In my US experience, it is the ENT that does microsuction but some lean to irrigation. Most emergency departments also use irrigation or disposable instruments.

    Yes. Only ENTs do suction in America. But I could call and get an appointment in 2 to 3 days. Maybe even the same day. We do not have the wait times like you guys do.

    Not sure where you patient lives in the US but I have no problems getting an ENT appointment. My first cleaning was done by my family doctor's nurse in Florida; she did it with water and I instantly got vertigo. I told myself I would go to a ENT Dr. I relocated to Wisconsin and made an appointment and only had to wait a few weeks. He used suction besides other tools. He retired and went to the ENT Dr he was referring his patients to. New doctor asked what I would prefer suction/water. I go 2 - 3 times a year for a cleaning. Thoroughly enjoy your videos.

    Yup! No microsuction for us here in the US--at least not that I've found. My UK husband spent a lot of time in Wales as a child, and when we visit his family again, we would like to rent a caravan in Wales. If we do, I am going to make appointments with y'all for my husband and my younger son to get a good suction on their lug holes-- LOL! I'm the waxie fan; our son will want to see what he produces, but my husband would need a nausea -related receptacle if you showed him the fruits of your labor. 🥰🤩🤣

    Honestly the thing about appointments applies to just about every single visit besides your yearly checkup or emergency visit. Anyone not in general practice is a specialist. And 90% of the time, you are expected to navigate setting up these appointments and communicating between offices yourself. Its why our emergency rooms are always packed as fuck, most people dont bother even if they can afford the costs and try home remedies until things get bad enough to go to the ER.

      Also 2 hours is a LOT longer for yall than it is for us. The US is way more spread out; some people take 2 hours to get to WORK every day. Like yes, 2 hours is a long while, but for context my family regularly took 6-hour drives to visit family multiple times per year.

    What???? My mom and dad both had micro-suction done with no problem here in the US, and they didn't live in a big town. It was a physician assistant in the ENT office.

    We use ear lavages in primary care, if you get an ENT you get the full treatment.
    Audiologist can remove wax in the U.S. and it’s not hard to get an ENT appointment. Your primary puts a consult in and ENT calls you get in and see them. It’s not HARD TO GET INTO ENT(Ear Nose and Throat).

    My son recently had and ear problem that was extremely painful. He went to Urgent Care and they shot water into his ear and caused it to bleed. When's he went to and ENT Dr he gave him antibiotic. He says its better but we don't know why his ear had the problem to begin with. Our medical care have become very convoluted I feel. Love your channel.

    I have the same experience when the wax plug comes out of my ears. Sounds are so loud it's actually physically tiring for my ears. I have stay in a quiet space for about a day. Even my pants swishing together is annoyingly loud. Once that passes it's just amazing.
    In the US we have to get a referral from a physician to see a specialist in order for insurance to cover it. Even then I've never had the small suction used. It was either the spoons, hooks or a very small irrigation, vacuum combo tool.

    GREAT story and video. I have a friend with MS who has developed cholesteatomaand needs cleaning every 6 months and the cleaning is done with water. It doesn’t hurt but makes her cry. She hates it. It is a large medical center. So sad.

    Yes, irrigation is what they usually do in the USA. I have had 2 of them in the early and late 1980s. Nobody ever suggestion the suction devices. Make sure they warm the water up.
    Oh, the person I went to was a Ear, Nose & Thought physician.
