Have you tried a squeeze bottle for contact cement? I use those for my foam stuff and it makes it a lot easier

For second mistake you could also dremel the edge of knee cap, but with that type of connection I always put hot glue on the back.

I have a question how do you know what millimeter of foam do you need to use?

But you don't explain how you know you've scaled the model part correctly. Sometimes even though it covers the avatars body part, it still isnt going to fit right. It could be too big. That's the tutorial I needed, anyone can pull on handles until it looks good, i needed something a little more comprehensive...
