This is a sequel to Eminem's song Stan on the first Marshall Mathers LP. "Stan" is about a crazed fan who kills himself, his girlfriend and their unborn child because of Eminem's "lack of attention" paid to him as a fan. 'Bad Guy' is about Stan's little brother who loved his brother more than anything. Matthew Mitchell is all grown up now and wants revenge for Stan's death.

This is the the best explination I've come across...
For the people who don't understand what this song is about, it is about the old song "Stan" and stans little fictionary brother, "matthew mitchell" getting revenge on marshall by murdering him. (Read below)
There are 3 voices in this track. The first one(from 0:00 to 5:10) is Matthew's voice describing such deep dark feelings on marshall. Notice in the song "stan" quote "Here's an autograph for your brother, I wrote it on a starter cap." That starter cap is the same cap as the broncos one in this song, quote.. "And here's your Bronco hat, you can have that shit back cuz they SUCK!".
So the second voice starts at 5:12 as a hybrid, merging with eminems conscious and describes the way eminem is dying just like stan did. This part of the song is the most critical, shocking, and epical. This verse made this song what it is and also got many peoples' interests.
The third and final voice at 6:50 is by eminem himself which he is trying to explain why he does what he does and why he needs to keep doing it. But the voice is quiet and then soon fades away.
This song really touched my heart, I never knew Eminem could make such an emotional master piece and amazing fiction storyline. His lyrics are super incredible and every single word that came out of his mouth were stunning. It is almost like this story is real.... ;)

This album came out when I was in 11th grade and it was the best thing I’d heard from Eminem at that point. MMLP2 is one of the greatest albums of all time. He transcended rap and went into storytelling and art.
Mf made a sequel to a song that came out when I was like 5 and did it perfectly. Eminem isn’t just a rapper, he’s a poet and a story teller.

"Stan" is a term that got taken *from* Eminem and used to describe an obsessed fan. In the video for "Stan," it was indeed one person; Devin Sawa played the character. In this followup, Matthew is Stan's little brother "the little brother who waited for hours with a starter cap."
What he meant about Stan and Matthew being symbolic is that he used that story and scenario as a metaphor for the things that were going on in his life when he was getting shit on by main stream media and accosted by maniacal fans whenever he would step out in public. This is what ultimately led him to the more hardcore drugs that got him hooked for real.
So in one way, you're right; it is a term used to describe obsessed fans, but the borrowing of the term came as a result of the "Stan" song, not the other way around.
That dope ending verse is Em's conscience and regret talking to himself about all those great controversial diss tracks he was famous for, and the people he'd hurt because of them. Also talking to himself about how he's getting old in the game, and he thinks it's time to step down with this final album.

So I'm in high school , I'm a senior and right now we're reading Macbeth by William Shakespeare, and after I heard this I was comparing the two artists. Because when eminem said that Stan and Matthew are symbolic towards his life as a a rapper in this song. And my english teacher said that the character Macbeth was symbolic to William Shakespeare because he wrote the play for a king that he did not like and in the play Macbeth kills king Duncan to become king himself. And another comparison between the two is that they both had troubled marriages and both have/had two daughters. Just thought it was funny. So in case you are a little confused, in the song "STAN" by Eminem, a dangerously obsessed fan (Stan) got upset when Eminem didn't respond to his letters for a long time. The fan goes crazy and shoved his pregnant girlfriend in the trunk of his car and drove off a bridge. The fan had a little brother (Matthew) who Eminem sent an autographed hat. The main character Eminem takes in this song is Matthew, the old crazy fan's little brother. This is why he said to take the (autographed) broncos hat back. He had a bad life and he blames it all on Eminem for making his older brother go crazy and kill himself. So he kidnaps Eminem and he was going to bury him alive next to where his brother was buried but instead decides it would be more fitting to drive off the same bridge his brother drove off all those years ago. Then the song turns into something about Eminem righting himself for leaving his fans when he took so long to actually put out another good album.

This song is great, it's 4th verse's epic and the fanmade video clip exceptional. But sadness increases when you notice that the 4th verse describes exactly what Eminem is becoming.

I found this song after having nostalgia of Stan in my head so I went to RapGenius to read the lyrics about Stan only to have my mind blown about some of the shit I never knew about what the lyrics meant until now, then I read about this sequel to Stan and started listening to this and amazed at how Eminem calls back to his previous songs and continues the story like another movie and then when I hear the last verse on this song my mind exploded because I just realize the greatness of Eminem is so big that my little mind can't even wrap around how many secrets and meanings he puts into these songs that he can go back to and open them up again to continue the story on. Jesus christ, no one else in the game does what Eminem does.
