How do you go from slow and perfect, say 60bpm, to 138bpm and perfect?

Good question! The short answer is gradually increase the tempo. However, it is important that you are maintaining the smoothness as you increase. You will find that you hit some barriers like tension or wanting to rush through something, then you have to slow it down again and smooth those things out as well as give it some time to sink in.
