Narrow boat fail.............Motor and Butty, Alrewas river crossing mistake!

The butty slips the cross ropes at 1:51, things go pretty wrong from there out.
Really tense for a moment; glad to see you had the power to pull out.....
I was fishing there a few months back and watched 2 boats go thought the same situation where the river meets the canal can be nasty in flood. I dont own a boat, im just a local fisherman. I was nervous for them.
What exactly was the collision noise caused by? The camera boat appeared to have more than enough distance to the grassy canal side; I though it sounded like a separate narrowboat hitting the camera boat from astern; or, was it the trailing boat hitting the towing boat?
I dont know much about narrowboat. So I dont really understand what happened. A butty tow rope slipped out of position. From the camera angle the butty appeared to be listing to starboard and take on water? Or was that prop wash? Thank you for your patience with my ignorance.
Butty was on two ropes crossing the bow, this gives some passive steering from the motor boat (like articulation). One rope jumped the bow letting it swing round to one side. This coupled with the side pull from the weir made the butty list and pull the motor round. luckily the river was not so wide, quick knock on the bank and we were straight again. I must point out I was lucky and this could have turned out much worse.

Butty way too loose on back end !! Never tow upstream when gauge on caution , but if ya must, always walk ahead to visually check speed of flow and then stay in the middle .... I've been up that stretch on a full moon on red ! Though I've got 72hp and I wasn't towing ... still quite scary ! When I jumped off at the lock mooring I landed in a foot of water lol I kissed the lock when I was safely in it..
Why was the butty not being steered correctly and why was it not connected to the motor correctly?
The bow of the butty slipped one of the cross ropes as we rounded the weir at about the 1:50 mark.
We picked the butty up the weekend before, so fast river might not be the ideal proving ground.

I think she was trying to tell me about the extent of our situation, however I was fully aware we were in a mess!
