Yep, camera man flashes her with the camera in front of her, she isn't paying attention as she takes her next steps which take her over the metal grating, and at the same time just as she let's out her "Whoaaa" yell, there's what appears to be a volleyball or other round object bounce right past her foot and the camera man in front hops out of its path, and her right foot stumbles before anyone touches her but since she stops moving forward because she's stumbling back, the people behind her naturally push into her because they didn't stop and now it seems like they knock over Tom's wife instead of she just isn't watching where she's going. The dude with the backpack is the one to blame from start to finish. Pushing everyone to be in front. And then, he just disappears when Tom reacted. I feel bad for the kid and the dude with the sunglasses, he actually stops Tom’s wife from falling after he was just pushed.

Had Hanks done this, he would have discovered his wife actually tripped on that sewer grate when she stepped off to the right when that one guy stepped in front of her to take a picture. Then, a gentleman would simply say something like 'I am really grateful and appreciative for all your support, but please give my wife and I a little space.'
