COVID-19: CT Findings

    Recommended playback speed: 1.25 to 1.5
    Learning Objectives:
    -Identify and describe the spectrum of findings in viral pneumonia
    -Explain a basic algorithm for managing patients with suspected viral pneumonia in a radiology department
    Note: CT is not recommended for screening or diagnosis of COVID-19 pneumonia by The American College of Radiology or the Society of Thoracic Radiology. CT should only be used when indicated by the severity of the patients symptoms.
    Summary:
    The Findings are non-specific with CT showing a pattern of acute lung injury (organizing pneumonia)
    There is a broad differential diagnosis:
    -Viral pneumonia (Influenza A, influenza B, RSV, coronavirus, rhinovirus, parainfluenza, and adenovirus)
    -Bacteria pneumonia (Legionella, mycoplasma)
    -Drug induced (chemotherapy, immunotherapy)
    -Cryptogenic organizing pneumonia
    New consensus statement, directly quoted:
    " Essentials
    ■ Imaging is not indicated in patients with suspected COVID-19 and mild clinical features unless they are at risk for disease progression.
    ■ Imaging is indicated in a patient with COVID-19 and worsening respiratory status.
    ■ In a resource-constrained environment, imaging is indicated for medical triage of patients with suspected COVID-19 who present with moderate-severe clinical features and a high pre-test probability of disease"

    Clinical context is key with specificity dependent on a high pretest probability:
    Symptoms of COVID-19: Fever, shortness of breath, cough, fatigue, no rhinorrhea (runny nose)
    Please Wash Your Hands (for 20 seconds and more frequently than in the past)
    Do Not Touch Your Face (Touching a contaminated surface and then you face is a primary route of spread).
    If you are over 60 y/o (or under 60 with high blood pressure, diabetes, or cardiovascular disease), consider staying at home, and avoiding groups, crowds, and travel.

    I live in an area where Covid positivity rate is 0.56% with an estimated CFR of 1.02%. Symptoms experienced by me: Fever, Cough, Ageusia, Parosmia, Diarrhea, skin rash, hypoxia with cyanosis. My RTPCR came back negative. HRCT scan showed diffuse bilateral ground glass opacities mostly on the lower lobe and peribronchial distribution. It also showed a thin layer of pneumothorax with multiple lung nodules. Can it be covid 19 pneumonia?

    Thanks for this nice and descriptive presentation 🌼
    But, i have some questions:
    1- can ct be considered as a prognostic tool ?
    Many patients show complete remission of symptoms and turn -ve for pcr; yet their ct findings remain present
    And so the question is can we discharge the patient in these circumstances?
    2- on the other hand, many patients demonstrate +ve pcr with -ve symptoms and -ve ct findings
    Is there a possibility that ct findings may be delayed ?
    And so the question is do i need to repeat the ct? And after how much time? And how frequent?
    3- many patients with typical ct findings, doing it initially for reassurance, have actually no symptoms at all, no signs, no lab evidence of infection and even -ve pcr for covid-19
    And the question is does ct be considered as an evidence to start isolation ? Or treatment?

      if you suspect you have it try eating things with potassium. that is what i have been doing, idk if it will help me alot. but i have seen that hypokalemia is one of the cases of death of corona virus. meaning low potassium in the body.

    Is it possible to see the ground glass opacity in the chest x ray?

      It is possible to see the ground glass opacity on the chest x-ray. However chest x-ray has not shown to be sensitive for ground glass opacities. Therefore in many cases the chest x-ray will be normal and a concomitant chest CT can be abnormal

    Ihave seen in ct impression is a few faces of ogg in peripheral of both lower respiratory lung loap.ct take n on14/7/20 immediately they have started Dexamethasone injection one's a day with ceftrioxone 1gm twice a day for 5days.day 3 is completed.followed by Azthiromycin 500mg one's daily for 10 days.some time dry cough is coming.please suggest shall I go for further treatment or that is enough.from india

      I do have some chest x-ray imaging, however it is not specific or sensitive for the findings of viral pneumonia. It is therefore not very helpful in terms of educational value.

    On 4/6
    Tested negative to covid
    Lung X-ray -said no pneumonia
    But this was my ct scan result
    LUNGS AND PLEURA: No pleural effusion or pneumothorax. Mild basal
atelectasis. Irregular/spiculated consolidative opacities in the medial
right upper lobe abutting the medial pleural surface measuring
approximately 3.6 cm on axial image 30. There is mild surrounding
groundglass. Minimal patchy groundglass and the lateral right upper lobe
on axial image 36.
