Port 3074 (UDP and TCP)
Port 3478 (UDP and TCP)
Port 3479 (UDP and TCP)
Port 3480 (UDP and TCP)
Port-forwarding on PS5 is configured via the router.

Same here. I've tried rebooting my router and entering the ports individually as well, same issue.
