    This animated video explains the anatomy of the Rectus Femoris muscle and hip pain that is caused by strain of the rectus femoris muscle. This condition usually occurs in active individuals and runners. The action of the rectus femoris muscle is explained. Palpation of the rectus femoris muscle and contraction test of the rectus femoris muscle is explained. Injection of the rectus femoris is mentioned. Palpation of the rectus femoris is outlined. The treatment of rectus femoris tear is outlined, stretching exercise and active release technique.
    Hip Pain is strain of the rectus femoris and its anatomy.
    One of the causes of hip pain that is usually forgotten is a strain of the rectus femoris muscle and tendon.
    It causes groin pain or anterior hip pain in some athletes.
    Who are these athletes that gets problems from rectus femoris strain?
    - Runners.
    - Baseball playoffs.
    - Hocky players.
    - Soccer players.
    It will be sudden like kicking the ball in soccer or sprinting from a standing position.
    It’s an overuse problem with repeated tearing and repeated stress and stretching of the tendon.
    There will be some scarring, adhesion, or tightness.
    Strain of the rectus femoris is similar to a tennis elbow type of problem or Achilles tendonitis.
    It is important to know that there is a lot of causes of hip pain, especially the anterior hip pain, it can be something inside the joint:
    • Loose body.
    • liberal tear
    • Avascular necrosis of the femoral head.
    • Arthritis.
    Or something outside the joint:
    • The illiopsoas bursitis.
    When you work the patient up for hip pain, make sure to include the rectus femoris muscle strain, which occur from tearing of the muscle fibers due to stretching of this muscle.
    The rectus femoris is the anterior muscle of the quadriceps femoris.
    The quadriceps femoris is a group of four muscles on the anterior thigh.
    The rectus femoris has two heads, originating from the pelvis:
    1- The straight head arises from the anterior inferior iliac spine.
    2- The reflected head arises from the groove superior to the acetabulum.
    The rectus femoris muscle is inserted to the superior border if the patella through the common quadriceps tendon.
    This muscle flexes the thigh at the hip and it extends the knee.
    The innervation comes from the femoral nerve: L2, L3, and L4.
    When the muscle crosses the hip anteriorly, it flexes the hip.
    Tears or strains of the rectus femoris muscle can be an acute process from forcible acentric contraction of the muscle.
    These injuries are usually more distal on the thigh or near the knee.
    Patient with a strain will have pain in the groin area or anterior part of the hip, most commonly you will find that the pain is more distal.
    The chronic overuse injury will give you anterior hip pain, usually near the anterior inferior iliac spine.
    In adolescents, you will have avulsion of the anterior inferior iliac spine.
    And the treatment is no surgery, just some rest and crutches.
    Palpate the site of the pain.
    Go around the hip region and palpate the anterior superior iliac spine.
    The ASIS is where the Sartorius is attached too.
    Immidiatly distal to that you find the soft spot or soft groove followed by the anterior inferior iliac spine, you try to palpate carefully because It maybe the area of the rectus femoris sprain if this area is painful.
    Also you want to palpate the tensor fascia Lata and the iliotibial band around the hip area.
    Try to palpate the rectus femoris muscle through its entire length because you may find pain in the mid-thigh or distally (common).
    Then you will do the provocative test: resisted hip flexion will produce pain.
    This situation is different then pulled groin which would be abductor muscle strain and the patient would have pain and decreased strength with resisted leg abduction, and it's called compression contraction test.
    So we finish the exam for the rectus femoris muscle strain then we will probably not need any x-rays, but in adolescents we may get an x-ray to make sure that there is no avulsion fracture of the anterior inferior iliac spine.
    If you get an X-ray probably it will be normal.
    Get an MRI I tough cases that are not getting better with treatment.
    Treatment is done by:
    - Ice
    - Rest
    - NSAIDS
    - Physical therapy: stretching and strengthening, active release techniques.
    - Injection
    Surgery is really done.

    I'm a massage therapist and have had a least 4 client with pain in the groin. Some that are older that need hip surgery. Others clients are younger and are athletic. I've had all sort of people my PCP, the chiro that I work with and a physical therapist...but I think your explanation appears to be the one I'm looking for.     Thank you so much for this video. I am 54 years old and had hip replacement on my right side 10 years ago. I have had pain in my right leg, just above the knee cap ever since recovering from the surgery. I have been to physical therapy many times and told that I have permanent damage to the muscles surrounding my hip as a result of the surgery. I try to keep my pain at a manageable level by keeping my weight down and exercising. Even when doing this, my daily pain level is a 3-4 with spikes if I catch my foot. I have been falling more and more lately. I catch my foot on something and then the pain shoots through my leg and I fall. On my own, I recently have begun my own research and I the pain I feel is coming from the Rectus Femoris just above the knee or the Common Quad Tendon just below where it attaches to the Rectus Femoris. Any suggestion on how to continue reducing the pain would be appreciated. I am in otherwise great health, have great blood work, and take no medications.

    I’ve had hip impingement surgery and hip avulsion fracture surgery back in 2021, I did some physical therapy but my rectus femoris still seems to be weak. I’ve tried laser therapy, cupping and scrapping but it only gives me a temporary relief. It hurts when I sprint and squat. Left leg feels 100% and my right leg feels 70%.

    Got this issue 1 year ago on right leg probably due to high volume Squat. Can't recall how long it takes to recovery(Probabaly 1 month or more), it just disappear one day... Unfortunately Got this issue again on left leg 2 weeks ago...probably due to Squat(again) and high frequency Cycling(Everyday ...) ...I won't call it pain but some weird soreness, have to stop squat n cycling for a while again. Light weight Ecc-foucus Leg extension and foam roller/Lax ball massage help a lot I think.

    I did a pelvic MR neurography and the medical report says "There is a nearly complete gluteus medius tendon rupture at the myotendinous junction" but my symptoms are pain at the coccyx part when sitting and doing physical activity. Could this be related, can this be the cause of pain. 9 months ago I removed my coccyx though (full cocygectomy) and no improvement at all

    I came here because i believe I strained my rectus femoris. It's an overuse injury for me I think from intensely training with Leg Extensions and Leg Press, and other leg exercises for bodybuilding purposes.
    I believe since it's more on the lateral anterior side of the upper leg (with emphasis on the anterior side) more near the hips, it's the rectus femoris, the culprit I think is extreme bouts of leg extensions after heavy leg presses.
    I used to have similar pain on the lateral side of the upper leg (same leg) from squatting, stretches made it go away.

      It apears it was Hip Bursitis (Trochanteric Bursitis). And that radiated outwards to the front as well. Sometimes radiating pain even to the innertigh and lower back.
      I did go to a physical therapist and the real culprit was not clear from the get go because of the radiating pain symptoms. It only got worse during therapy. Than I had an MRI scan that made it clear it was Trochanteric Bursitis and not something else.
      Then physical therapy got adjusted for treatment of Trochanteric Bursitis and eventually the pain went away.
      Make sure to go to a good knowledgable phsyical therapist though. I had one that was not that great and I was forced to switch to another physical therapist.

    i had two aiis avultion fractures then had a screw put in and taken out when i was younger recently same problems with that area i had an mri and the radiologist said it is a sprain and i have an oedema from the aiis along the rector femoris to the musculotendinous junction will it ever fully heal he told me to rest and start small exercises

    I believe I have snapping hip syndrome from this muscle but they insist it’s the IT band. But the severe pain and tightness is not on complete side it’s more partly side. Lateral but just a tad bit back from much research the pain and tightness is right smack the satorius.

    I am a semi pro soccer player and I have a complete tear to my proximal rectus femoris at the musculotendinous junction with 3-4 Cm gap, after 3 months since the injury would you think surgery is necessary being i still want to play at a high level

      Bro i also have had grade 2 or 3(complete) tear to my proximal rectus femoris. I was shooting like Ronaldo's free kicks.... maybe u also?Haha
      Now it has been 2 years. i overused my legs and my hipflexor has been inflammed 6 monthes now....
      Any advise?

      @@ashuraya2779 I did my right by taking a free kick as well hahahah wow. my left one which is a complete tear was from a finishing session, I've been doing rehab for 7 months now which has been strengthening all the muscles around it and I've seen a sports therapist to to manual work. I would definitely go see a sports doctor if I were you.

    Hi Doc, I have a question. If I tried therapy but didn't work due to me rupturing both quads playing soccer how can I strengthen my quads or the tendons?

      I have had rectus femoris tear and i after some healing overused and my hip i hurning now..... what did u done please brother help

      @@ashuraya2779 You need to stay still as much as possible. Wait about a month and half of the tear and start therapy. Small movements, start training the hip to walk and take small lunges. No soccer for a year until quad is firm enough to lunge and sprint. Do a alot of physical therapy and plenty of hours of daily rest. Try not to bend knee so much, keep leg straight as much as possible for now.

      @@ashuraya2779 Also when showering, cold water as much as possible. After tendon starts to get hard again in 2 months, mix it up. Cold water then hot, then cold again. Repeat..I still do this for tendon swelling

      I can run 100% speed but my rectus femoris is tight and weaker. My second problem which became after overusing is hip.. maybe i have inflammtion

      Should i restart the healing prcess should i wait a month for the hip to heal ?
