my university is very late in issuing provisional bachelor's certificate (it can be August), so can I get a DOV just by showing my transcripts? or will I need to drop a year?

Ilets is not required only if you're studied your previous course in English...
For Milan Univ it is IELTS should be greator than 6 or equal to.
Your grades also matters because the University of Milan has 140 rank around world.
Sometimes , they see your proficiency also , your bachelor profeciency also counts .
So , its good if you have IELTS.
