    Why was there no histology in this histology video? I was so hoping to see some pathological sections!

    You missed the vascular endothelial glycocalyx layer.
    I believe the causative factor is a barrage of assaults on the glycocalyx, without time for recovery.
    this allows the other toxic risk factors access to the endothelium.
