    I almost laugh when it says pain may last for around an hour after waking up Mine never left. It was relentless. Shoulders, arms and upper legs all impacted.

    Up to 20% of people with Polymyalgia Rheumatica do not have elevated inflammatory markers.

      My wife has a normal ESR and only very slightly elevated CRP. Has confirmed GCA with a Temporal artery biopsy. Doctors were slow to treat with Prednisone because of the reliance on the ESR. All other symptoms, persistent headache, scalp tenderness, jaw and tongue claudication, pronounced temporal vessels, and visual disturbances were present. But it wasn't until she lost total vision in her right eye twice for about 10 minutes each time that they finally began the high dose steroid treatment. Don't just rely on the ESR like the emergency department doctor did.

      I’m VERY sorry she went through that….The MDs seem very reluctant to discuss the possibility of GCA…I have been there…

    PLEASE know that words cannot describe the pain and incapacity PMR causes. The pain does not really go away after an hour or so in the morning, you just finally get your mind to push you in spite of the pain so that you can function. I never showed elevated esr or any related marker. One thing to listen for is my experience, which is common-- I woke up one morning feeling as though I had been crushed by a steam roller. Every bit of me felt damaged and in pain.

      My one visit with a horrible rheumatologist...looking for an "official" diagnosis for my PCP, he asked me why I was already on steroids the last 6 months. (Lol) I asked him if he had any idea how painful this is. He said, "No I don't".

    Im dealing with this crap. Mostly back, thighs and shoulder blades. Just started prednisone and methotrexate a few days ago. Big relief already!

    My PMR went away when I stopped taking steroids and changed my diet. I stopped the intake of vegetable oils, ultra processed foods, sugar and cut down on carbohydrates. I used to get indigestion and that went too.
    Olive oil, oily fish (not farmed salmon and trout) and cut down any food that contained lectins also became part of my diet. It worked and today I feel fine (at 81) and go on regular long walks.

      I was diagnosed with PMR 12 mths ago & pain was high 8/10 in shoulders and had a 6 wk course of Prednisolone & pain went away within a couple days but I had to taper off it over 6 wks. After that I changed my diet a few times and was able to lower my pain to a low level 3/10 so I didn't need to go back on steroids. hasn't gone away completely still. I've cut sugars , carbs etc but any other advice on dietary recommendations or supplements?

      I struggled for a while trying to find something that was causing the problem and it took me a while to find out about the toxicity of vegetable oils and ultra-processed food. Vegetable oils are responsible for so much ill-health and there's no reason that PMR is not included.
      Vegetable oils can be found in many processed foods including sauce, fake cheese used in cheap pizza etc. It's better to avoid it all.
      I would also include lectins in a thing to avoid. Lectins are pert of a plants defense against being eaten. They are a tough spiked protein that clings to the gut wall and tears the gut wall allowing toxins to enter the body. They are found in wheat, nightshades and beans commonly but in beans they can be removed by soaking the beans before use.

      @@affposter3088 My PMR went away too after taking steroids but steroids have side effects like cataracts and they become less effective over time.
      I wanted a cure for my PMR and not a remedy.
      Once you figure out that these things don't just happen they have a cause you can then look around for the cause and then you can eliminate that cause.
      At the same time as my PMR I had cancer and AMD. The cancer has now gone and the AMD not progressed.

      ​@affposter3088 So did mine. However, as I steadily reduced the steroid dosage, the PMR returned. To eliminate the discomfort, however, I now take a couple of 500 mg paracetamol, and an hour later, I'm practically pain free. Ibuprofen also reduces the discomfort, but it's not as effective as paracetamol.

    TELL YOUR PATIENTS TO TRY The Carnivore Diet. This took all my PMR and Rheumatoid Arthritis symptoms away! Been 6 months now. I suffered with these diseases from 2017 until this May..I am symptom free now.!

      That's what I'm doing! These doctors never mention nutrition!. I came down with PMR middle of Feb 2024. Since my husband cured his diabetes with Carnivora he suggested I look into Carnivora for autoimmune disorder. So for the last 5 weeks I've been on strict carnivora. I messaged with some people what cured their PMR with Carnivora and most of them said it could take 3 to 6 months to feel better.

    Assuming the patient's symptoms are typical of everything else you've mentioned (i.e. upper arms, shoulder pain, hip girdle) then what if...
    3:52 They are also not able to do things with their hands (like do up buttons, turn a key in the door, use a mobile phone, type, sew, knit etc.), and...
    4:20 It also hurts for them to clench your hands?

    I am a retired internist. For two years I have been experiencing what I call migratory polymyalgia. My pain is mostly in my scapular area lasting a few to several minutes then disappears for hours or days. I have had the pain also in the muscles of the backs of my upper arms and in my lower back and rarely in my hip girdle area. Once the pain was in my mid torso more in the front. I have no weakness or stiffness. I have no skin involvement. I am grasping at straws to understand what this might be. It started about two years ago at which time I also developed very frequent PVC[s which led to my ejection fraction going to 25 percent and symptoms of heart failure, mainly fatigue.. After treatment with ablation and Entresto, my EF returned to normal. However, these myalgia pains have continued. Any idea what differential diagnosis list might be? Nothing seems to fit this pattern. My sed rate, crp, and cpk were normal as was my tsh. My labs were all normal other than a low vitamin D. I have wondered if that could be it. Any ideas would be appreciated. PMR has been at the top of my list with perhaps no abnormal inflammatory markers.

    My brother is an eye surgeon who has GCA patients. He stated, “the temporal artery biopsy for inflammation will NOT be of much use if the patient is ALREADY on prednisone…”

      I am a general practice registrar currently on a Rheumatology placement (UK). The consultant supervising me said something similar to your post - If the patient is started on steroids the inflammation would likely have already settled and the biopsy may come back negative. He mentioned that ultrasound of the artery is becoming more and more popular as they are less likely to miss the disease.

    Can you tell me “IF” someone could have PMR with a positive RF(92)? All other symptoms are classic clinical to PMR except male 58yo. No joint pain, swelling. Bilateral shoulder/hip to above knee extreme pain within 24-36hours

      A positive RF does not rule out PMR.

    How would you determine whether I have Psoriatic Arthritis or PMR? (I have shoulder stiffness in spades) but minimal impact from Psoriasis. Can a Water Fast (10 days) reset the immune system?

      Psoriatic arthritis has swelling of the hands and a rash. PMR is more stiffness and is seen in older people. I have never heard of a water fast and I would not recommend abstaining from water.

      Yes, I have heard that a water fasting of 7 to 21 days would get rid of several autoimmune disorders. I fast 3 days a week and eat ONLY once a day, meat only diet, 4 times a week.
