Well there's also the fact his mom is a medical nurse. A nurse or a doctor who works in the medical profession and sees numerous patients on a daily basis would know how to recognize the symptoms someone who is legitimately hurt or sick would have.
If your own mother works in that field, then she too would know how to recognize the same symptoms someone who legitimately has that issue and could tell if you were faking it or pretending. Of course Mikey didn't stop to think about that either.

She said running a mile in PE was the worst part of school but in first grade we would have a 20 feet straight and 25 feet across gym and we would have buddies but each Buddie would take turns and you would have to run a lap around the gym
