The way it works is that the buttons on aftermarket steering wheels connect to the steering wheel hub. The clock is static between these two pieces. The hub however will often have what's referred to as a clock spring or tab which rides against the back of the hub and allows you to have connection without ever worrying about wiring getting twisted.
