This is lovely - I have to try this. I wonder if instead of folding the page of the book at the spine end, would it be possible to just cut the pattern on a 45 degree angle at the place where the color meets up with the gray background before it is applied to the book? There would be no gray showing on the book top when completed. That way the design would just be lying on the page and would be the same look without the dimensional look without the cutting of the book page at the spine area.

Yes, that would certainly work! It just wouldn't have the dimension, but you can contour around the image.
How do you get such a tall photo that is tall enough for the front and the top? My photos are either square or rectangular. Thanks.
