  • This video describes the pathway of general sensation from face.
    - 3 divisions of trigeminal nerve (ophthalmic/V1, Maxillary/V2 and mandibular/v3) carry general sensation from face and pass through trigeminal ganglion. the first order neuron stop in trigeminal nerve sensory nuclei that extend through the length of brain stem:
    - Pain and temperature stop in spinal trigeminal nucleus/Medulla
    - touch stops in principal nucleus/Pons
    - Proprioception from tempromandibular joint carried by V3 and ends in mesencephalic nucleus/midbrain
    - Second order neuron arises from these nuclei and cross to the opposite side to ascend in brainstem as trigeminothalmic tract/trigeminal lemniscus that ends in the ventroposteromedial nucleus of thalamus (VPM).
    -3rd order neuron, arises from thalamus and ends in the lower part of sensory area (postcentral gyrus)
    *Injury of Trigeminal lemniscus/trigeminothalamic tract causes contralateral loss of facial sensation
    *Trigeminal neuralgia is sever kind of intermittent pain that triggered by touch (brushing teeth, shaving or applying make-up). more common in female. One common cause is aneurysm of superior cerebellar artery. Trigeminal neuralgia can be treated by microvascular decompreession of the ganglion, anti-convulsant such as tegretol and gamm knife.
