    Sciatica is irritation of the sciatic nerve. The most-common causes of sciatica that I see in my physical therapy clinic are a bulged/herniated lumbar disc, degenerative disc disease (DDD) and stenosis, and pirifomis syndrome (tightness/pain in the piriformis muscle).
    Your piriformis is a muscle that that moves your hip. It originates on the outer edge of your tailbone and inserts over onto the outside part of the hip (the greater trochanter). Your sciatic nerve exits out of your spinal column and passes beneath this muscle. If the piriformis is tight or irritated it can put stress on the sciatic nerve and cause sciatica symptoms.
    Common causes of piriformis syndrome and concomitant sciatica include sitting for long periods of time, increased pressure on the piriformis muscle (sitting on a wallet, sitting with your legs crossed), and piriformis muscle weakness.
    In order to alleviate sciatica caused by piriformis syndrome, your treatment approach must include three key elements. They are massaging this area, stretching the muscle, and strengthening the muscle. Each one of these individually can offer you some relief to a degree; however healing will be faster and more complete if you incorporate all three methods.
    MASSAGE (6:25): promotes tissue relaxation and fascial release to help the muscle move a little better. The best ways to massage the piriformis include a foam roller and/or a tennis ball.
    STRETCHING (9:25): another great way to decrease tension in the piriformis. In this video I take you through a progression of stretches to help alleviate tension in the tight piriformis.
    STRENGTHENING (13:20): oftentimes piriformis weakness can be a cause of piriformis syndrome. Contracting this muscle promotes blood flow into the area and can serve to “reset” the muscle if it’s in spasm. The best home exercises to strengthen your piriformis include clamshells and fire hydrants (quadruped external rotation).

    Dr. Jared, I cannot thank you enough for this video (among many). I have suffered from very serious hip and radiating pain down my right leg for months. My RMT thought I had some nerve problems; my chiro suggested that it was a piriformis problem, but neither provided the massage, stretches, and strengthening exercises that you so comprehensively provided in this one video! I tried all of your suggested massage, stretches and exercises just now and felt an instant relief! From the bottom of my heart, THANK YOU so VERY, VERY MUCH!

      @@toneandtighten My comment is coming not only from the pain I've been experiencing, but also from not diligently understanding the advice I've received. When personal trainers and therapists give me 'advice' and 'instructions', they're often not clear enough. The expectation is I do much of the 'homework' in my own time, yet that's difficult when I'm unsure about (1) what to do, (2) how to do it, and (3) what the exercises achieve. When my body's to be manipulated, I expect clarity. So coming across sources that make sense helps push me one step closer to managing my health independently. Whether or not your exercise are effective for my body remains to be seen; however, your video was clear, logical and well structure, therefore helping me to at least better understand the pain I'm going through.     Hi- I will try the fire hidrant technique and see it works for me. I have a bulging disc. I have been in pain for the past week and half. =( I was doing well and flared the piriformis muscle again...I had actually started working out and on a routine and now I have to wait and start from square one. Thank your for these exercises.

    I just wanted to Thank You Both for this information as well as Camile's demonstrations. I have suffered from Piriformis Syndrome for years. It has only been very recently that I have educated myself to find some relief that really helps the pain. It seems the first thing the doctors want to do is surgery, I don't want the surgery due to the risk involved. I have learned that a lot of my problems came from 23 years of being a Professional Truck Driver and Certified Trainer for teaching other women to learn to drive as well. I had to retire from trucking due to the damage it has caused in my back.
    I did have to have major surgery on my back about 7 years ago, I would not recommend this type of surgery (Laminectomy Surgery). I will say that now they have a lot more Non-Evasive surgery and procedures where you don't wake up with a nine-inch scar down your back. I do have other issues that you discussed in one of your other videos as well, such as spinal stenosis, degenerative disc disease, bulging and herniated disc in the plural.
    You are one of the first who has actually hit the nail on the head when you said the pain can be debilitating. I have injections in my spine and now getting ready to undergo nerve ablation's again due to the nerves growing back again.
    My Question for you is this...I have heard of a procedure where they can go in Non-Evasivley and ablate the sciatic nerve under the piriformis muscle, however, I seem to be having a problem finding a doctor locally that can perform this procedure. Do you have any recommendations as to whether it would be an Orthopedic doctor or a Neurologist that would/could do the procedure? Any information would be helpful.
    Note: I have been doing the exercises that you and your wife showed in this video and surprisingly it has helped a lot more than I had expected. Day by Day, One step at a time it seems to be improving. Since I have been doing this, I haven't had the extreme spasms that cause my foot to kick outward.

    I'm in my mid 60s, and have a fair knowledge of anatomy. I ran into your videos by error, and I'm happy I did!! I learned soooo much about anatomy and now I fully understand my pain and where it's coming from. I've been able to identify the spots that are making me miserable. I'm going to be diligent and make friends with these spots that cause me pain and work with it to feel like a human being, and enjoy a pain free life again. This knowledge helps me to fully understand what is happening to my limbs and, instead of cursing it, make friends with it to help IT heal so that we can both be happy again....     Excellent set of videos on sciatica Dr. Beckstrand!! Truly appreciated especially after my recent visit to an orthopedic surgeon that diagnosed me with sciatica at 66. As an avid inclined treadmill walker, I thought I was dealing with tendonitis or IT band syndrome. I use to think only "old" people get sciatica ;-) The orthopedic surgeon prescribed a steroid pack along with oral diclofenac however I'm holding off taking it as I want to try a natural approach first to resolve the cause not the symptom. She also suggested I go to PT which is why I landed on your CZcams channel. After looking into the PT room at this orthopedic clinic, I really did not want to be in a room full of unmasked people during these times of covid even though I am triple Moderna vaxed. Your videos are extremely helpful especially in these times of covid.
    After watching your diagnostic video on sciatica, I truly believe I have piriformas syndrome of the right leg. I did a few of your stretching and strengthening exercises and I actually feel better in just one day! I'm still an active part-time employee (43 years) of UPS so I am not sedentary. I look forward to continued health thanks to your videos.
    P.S. Thank your beautiful wife Mrs. Beck-strand for this video also ! You two obviously have great chemistry together and I wish the both of you and your growing family all the best. FWIW, I've been with my beautiful wife (R.N.) now for 45 years and have two wonderful children from our marriage together. Our daughter has her Ph.D. in Genetics (SMA - Spinal Muscular Atrophy) and is now a professor and our son is a young 4th year medical resident specializing in Drug Addiction Psychiatry. He also plans on doing a fellowship in geriatric psychiatry which will be good considering the age of his parents ;-)

    Thank you so much for this video. I do have problems with my discs but this pain was different and started in my butt instead of my back. I couldn’t walk or stand up this morning and then I did the tennis ball massage. I can’t tell you how much relief it gave me. This started when I was pregnant and I was just constantly told it was sciatica and they couldn’t do anything. Thank you for explaining it well too, because now I understand it better!

    Your videos are talking straight to me! I was diagnosed at 30 years old with congenital spinal stenosis and I've dealt with pain for 25 years since. I've never been so bad to need surgery but I've had to change my whole lifestyle and it can be depressing sometimes. My workouts dwindled down from running 5Ks and weightlifting to light walking and riding a bike. I haven't done any body work outs in years because of the repercussions I suffer in pain for weeks after. I was looking for some stenosis stretches and I came across your videos. After completing them I found your low back and core exercises and was ecstatic to find movements that do not aggravate my condition. They are so effective! My goal is to keep my back and core as strong and flexible as long as I can to avoid laminectomy. I also follow your tips on sleeping with pillows and it's tremendously helped my back and shoulder pain at night. You are an incredible DPT!

    I want to thank you for the these videos…. I’ve been suffering from sciatic nerve pain for years, I was even seen by PT via my primary care physicians recommendation ( the symptoms went away during our sessions) however, I was informed that there were different types of sciatic nerve causes, until I saw your video to self diagnose at home! This is a game changer for me, because now I can focus on the right kind of exercise and strengthening and stretches for my specific kind of nerve pain ( periformis) thank you again, you have changed my life forever! 🙏🏽

    Thanks so much!!! I've gone to local physical therapists, and each one gave me different exercises to do that were very helpful. Love what you've shared in your videos. I'm 74, don't drive, and it's difficult to get out for physical therapy when my sciatica flares. Looking forward to trying that roller! 👍❤️🙏

    I finally understand why I have had hip pain and sciatica for almost a year now! In my case, it’s a weak piriformis muscle from too much hip opening and cross legged sitting (I’m a yoga teacher). Thank you, this helped so much!

    I have suffered from severe lower back pain and tightness in my muscles around my hips and pelvis which was then causing sciatica. I watched your sciatica video which led me to this video.     Thank you so much for the info. I'm 21 and I've been dealing with the pain since I was 16. Most days I have to call into work cus I flat out cant bear weight on my legs they either go numb same with my arms out of the blue. And I've seen doc after doc and you've gave me more info then they did. I'm going to wait till my roommates get home to help. Thank you again.

    I just tried the four stretches and tennis ball trick and felt INSTANT relief. I can hardly believe it! THANK YOU!     Thanks Doc! Great information and great demonstration! Incidentally, acupuncture does great things with any of the issues causing “sciatica” no matter what including herniations! These wonderful exercises along with acupuncture and PT or chiro will get rid of the problem quickly …just dont wait too long before getting treatment or starting these great exercises!

I've been trying to figure out why I have so much back pain that comes and goes and your explanation and tips for self-diagnosing are simple and helpful. I now know why I have sciatica pain (Piriformis Syndrome) and exercises and stretches to help. OMG, immediate relief! It still hurts a little, but it feels much better.     Soooo glad I found your channel! I was reviewing a video from my mom’s physiotherapist to help her. Then I saw this on the feed which reminded me of all the physio treatments I used to received. I didn’t remember them all, so it was great how you:
    1. Explained how we get it
    2. What it is
    3. How to reduce the pain in different moderations

    Thank you so much for this video. My sciatic nerve issue started around 3/2/19. I was propping myself up with pillows to breath better at night because of my allergies. Since then my doctor helped me with the allergies. However I had back pain. I was laying flat on my back around the first of March and I sneezed. My sciatic nerve slammed me into unbelievable pain. My lower leg and foot burned to a point it made me scream. I couldn't walk for about 4 weeks. My doctor and physical therapist helped, but no one mentioned what you taught me in this video. I truly believe it was this muscle that I injured when the back pain began. Now , 7/16/19 I can walk normally with no back ,hip or butt pain. I don't limp anymore. I'm waiting for my foot and toes to finish healing. I feel I'm on my way to recovery thanks to your information. Thanks again! Very educational.

    Great info. In my case I was told the cause to be sleeping on my side with the top leg in a hook position and another strengthening exercise to use would be a glute bridge. Thanks again.

    Thank you. Please don't stop helping others. After a year of looking for pain relief exercise, I gave up. I have a Pet Grooming and Wellness Spa. We help hurt animals and elderly. But us humans are suffering and I cannot imagine giving up on my Fur Clients, so your video has helped me where no others have and with just 1 time trying these. This will be in my favorites. 🙏🏼

    I've had a sciatica flare up and this video proved to be so informative with Easy instructions to follow. I will continue to do these exercises both to stretch out the muscle and to strengthen it. I am also using both a heating pad and and ice pack and alternating between the two of them to alleviate inflammation and to help with blood flow through the area.

    I just wanna take the time to say thank you. I have been suffering with the pain of a herniated disc for about 9 months now. I have been going to physical therapy for it, and though it helped out a lot this video has helped me relive so much of my pain. Although I’m not pregnant, that particular exercise helped me the most. Thank you

    I just tried some of those stretches and although it does still hurt, it doesn't hurt as bad. It really made a difference. With more practice I have no doubt that it will alleviate the pain
