I recently did Skyrim's Quest for Yamarz, and mine bugged out rather spectacularly. Yamarz got hit by the Giant's stomp attack and it launched him up and over the mountains surrounding the shrine the Giants had overtaken. But he didnt die on impact so he was stuck where my character couldnt get.

5:30 What's up with these heads, Oblivion, Starfield, Cyberpunk all share same problem. I think they are using some common algorithms for every animation, engine sets point A and point B for head, and animation chosen, but there is also inertia, physics applied depending on mass of head for example, when he stretch neck one side, but there are only two points in set A and B it circles around instead go back the other direction. Just like force of inertia was higher than "force" set by animation to go back B to A.
I mean all are just matrices of transformation, i don't know much about 3D graphics, i only know that every rotation, of object, head in this example, is basically result of all vectors of object, being multiplied by matrix of desired transformation. If this is animation of rotation it meast there is set of matrices, predefined to do 360' rotation, step by step. Depending on how fast this rotation need to be, you index this set, step by step, and you limit this rotation, speed of this transformation, indexing, is controlled by simple equation, some physical rule applied, inertia, to go smoothly, if data is corrupted, head instead of being limited continue iterate trough set of transformations and rotate, normally you would have limited angle your head can turn around, but because character can rotate body in every direction, instead of transforming this set of animation matricess every time, they are indexing 180' rotation pick min, max left right index, when limit is corrupted, it looses loimit.. I totally get this idea of my ass, i "feel" in my guts that reason we see glithes is because they all use simmilar trick for better performance, all is just fine until data get corrupted, one number is lost and.. head rotate 180 iterate trough set taht was associated.
Its called procedural animation, it's a way to combine bits of animation to become one so it's easier for developers, no need to mocap every single dialogue for every character
