How do you get Mania difficulty?
Like I've completed Hardest difficulty in Streets of Rage 2 with Axel and Max, but it didn't happened anything.

In the main menu. Hold down (A) + (B) buttons on controller 2 while selecting and entering "Options" with controller 1.
When it's done right, you can select how many lives you start with (up to 9), choose your starting stage, and play at the Very Easy and Mania difficulty settings.
