People are so dense. I think Joan is hilarious but I can totally see how that would be really annoying as hell and how it affects someone self-esteem. Mariah says the quote correctly but as continued the story she made it sound like Joan said something hurtful (Joan is great at roasting and reading so seemed reasonable) so I thought the clip was gonna show Joan being sarcastic Joan. For all of you who didn't get it, Mariah didn't say that Joan said she gained weight. Mariah quotes Joan correctly. However, she's making a little sarcastic joke because Joan's question made her feel insecure with her body. Why would anyone care what Mariah looked like when she's receiving an award for her song (I know she didn't write When You Believe but still). On top of making Mariah feel insecure at an event where she should feel joy, that question and those little comments can still be hurtful if you read between the lines. Joan Rivers implied that EVERYONE else think Mariah gained weight but she didn't think so. It's similar (not same) to the sentiment of "You're not like other girls" and things alike. Plus, Joan Rivers is implying that gaining weight is inherently bad from her question and statement. I like Joan Rivers comedy but sometimes her words can hurt people whether she means it or not.
No. What Mariah quoted was "i heard she gained weight. You look great to me," which is totally different from "Everyone said she gained weight, you did not." The former sounds backhanded, the latter isn't.

Based on Joan’s history of shading people and bringing up gossip at public events, I do not blame Mariah for viewing the fact that by Joan specifically bringing this topic up that she was not passively implying Mariah gained weight. Woman to woman, why bring it up❓ It's a touchy subject anyway, but to bring it up on the spot on live TV❓ The shock of it alone makes the brain only let's you hear the subjects you want to guard yourself against, the insecurities if you like. If Mariah wasn't caught off guard I can't help but feel she would not have "danced to the beat of Joan's drum" and twirled around so Joan could judge her all over, on live TV mind, so everyone at home too could focus on Mariah's figure, drawing everyone to focus on her weight. Mariah had no deflection clapback as she usually comes up with in a heartbeat in TV interviews etc, I was expecting her to say " why don't YOU spin around Joan, if you can without your walking frame for stability? " 😂 That would've spun it right back at her (joking of course, I liked Joan, but sometimes she approached things because she shouldn't.
