that's actually part of the original Demo 4 version. The theory is that after the line "SOME ONE HELP ME", it cuts to the scream before then going towards the original verse of Demo 4 which shares the leimotif of Labyrinth, which then continues into the song

The reversed part (there’s a comment labelling all the important stuff in it!) seems to be two families having a fight. But they do talk about the ocean and war a lot in this as well. When I only listened to the final version of the song, I had my own theory of it being about a *minor* sentenced to the electric chair or electroshock therapy. I thought it was this little kid's trial, with various people of the jury crying or interrupting. I swear I heard people going "just a little boy!" and "He's been known to-" in the final version at the beginning. This demo has made me consider something else, however.
the "I really have no power over my mind" guy is clearly the same voice as the "crying guy" from the final version, so it's strange that he's the one saying he has no control over his mind if the minor is on trial, which he sounds nothing like.
so i have a theory
he's the actual defendant, and the little boy/girl's voice is some witness or his kid or something making up a story to defend the guy. the judge saying "Oh?" prompting a response from the small child and it's response of "jumping on the beach of Timbuktu" sounds very made up and exactly what a kid would do to get out of trouble. it's immediately shut down with l o g i c and f a c t s, and the kid just says oh. the judge then comforts the kid with the "at least you tried, I'll give that imagination..." and I can assume he just called it's imagination impressive.
i have no idea what the genius soothsayer diviners are about, nor the "ok compadre" xd
as for the "You ought to be ashamed of yourself", it starts with a fade in and ends with a fade out, as if someone's saying this as we're walking by. as the song is meant to be a first person view, it's not unlikely that this is some jury member insulting the defendant as they walk by. the demonic and honestly creepily slow "Y O U O U G H T T O B E" might be how it sounded to our insane protagonist.
this is all just pieced together from blurbs from this song and my theory of the final version, so it's kinda half assed.
In short, guy goes on trial, pleads insanity, some kid witness makes up a story to keep him out of jail, but his story is obviously fake so it does nothing. Guy gets sentenced to electric chair or electroshock therapy anyway, and the final version of TME happens with stuff like lightning striking his temples, electrifying his chambers and scorching out his sovereignty.

I think "Thy genius sates a thirst for trouble" is him realizing it was a mistake pretending to be insane. His 'great idea' of pleading insanity to escape a prison sentence just caused him more trouble, actually going insane.

I used this as the background ambiance for a Mind Flayer hive mind in D&D, with the voices being the thralls they had planted in the city above. It was awesome, and my party loved it.
I use the version with Lyrics for an Artificer Lich who put his mind into his machines and became a living factory.
Heart, Mind, and Soul are all in a war. With the body discarded, his human heart is preserved deep in the factory, his soul within his phylactery and his diseased mind trapped in the factory with no conscience or morality.

The entire section is basically labyrinth. The synth playing through that part is just the labyrinth synth pitch changed and the bass is a altered version of the labyrinth synth bass.

I actually believe it’s “fi(fee), fi, fo(foe), fum” which could be alluding to to the story “Jack and the Beanstalk. The giant says this a lot, so the “fi fi fo fum” could be an adult or court official if you look at from the song’s perspective. (Yeah I’m way too into this song lol)
