      Moreover, before injecting, when you are tapping the syringe and removing the bubbles, that process also activates them.

    Hello doctor,very nice demonstration, don't you need any activator for your platelet before the injections such as CaCl²?Many say that if platelets are not activated it is pointless to use just blood because growth factors can not start do their job. Don't you agree?

      It is completely okay not to use any activator. Platelets get activated automatically as soon as they are injected into the scalp.

      - Prescriptions include Multi-Vitamin once daily or Vitamin D once every 15 days or once every month. And Topical Hair Sprays containing Minoxidil a few puffs on scalp 3 to 4 times daily.
      - One session per month for 4 months. Then one session per 3 months for next few times. Then shift to one session per six months, etc.

When I got mine done , I didn't get anesthesia, just needle in scalp, and some of the plasma was sprayed on the scalp instead of injected. i use minoxidil and other vitamins, but is once every month better than every two months?

      You are most welcome. Local anesthesia as shown in the video blocks the "supra-orbital nerves". These nerves go up and supply the scalp skin. Therefore, blocking these nerves blocks the pain sensation in the area supplied by them.

    It is impossible to derive that much true PRP from 30 ML blood draw. What this is producing is platelet poor plasma with a small amount of PRP mixed in.
    The amount of true PRP/Buffy Coat would be maybe 3-4 ml at best with this technology.

      No issue. It is okay if platelets are destroyed or activated right at the time of injection. Growth factors inside them are released and they work. However, before injecting, care should be taken to use wide-bore needles only.

      Cream cannot penetrate that deep in the skin. Therefore, injection is required.
      Note: Some people choose PRP to be done without any Local Anesthesia. Because pain is only mild and bearable.

    great video bu i have a question. In second spin, i thought that platelets stuck down with the remaining rbc's(a white substence), and the remaining plasma is the ppp. You discard some ppp and then you shake he remaining ppp with the platelets and rbc's, and you have your prp, wich is a litle red. Do i miss something?

      It's okay if ppp is not removed from the top. Total number of platelets injected still remain almost the same. RBCs should be removed as much as possible. They should not be mixed with PRP by shaking.

    is balancing the plasma vial with a water vial in the centrifuge actually accurate? yes you are trying to make the volume of the water vial equal to the plasma vial, but isn’t it technically lighter in weight than plasma? therefore, it’s not balanced in the centrifuge?

      Slight variation in weight does not cause any problem. If the levels are very off from each other, only then centrifugation gets difficult.

      2) notch is pressed so that lignocaine remains in the area injected and does not go to the loose areas nearby, like eyelids, etc. If lignocaine remains in the injected area for long enough, its effect will be maximum.

      Effect starts within a month. And lasts as long as PRP and adjuvant therapies (Minoxidil, Multi-Vitamins, etc) are continued.
      Note: All these therapies slow down the process of hair fall and alopecia. Currently, no therapy can actually prevent or stop it.

    Hi, I have a question. I have EDTA tubes (anticoagulation). If I draw blood into this tube and let it stay vertically for 2-3 hours, you get automatically a separation by weight into blood cells and plasma/platelet, Isn't it an alternative option for someone who does not have a centrifuge? The result of the tubes looks the same as with centrifuge. Thanks in advance.

      Yes. You can get plasma in that way too. But that takes a great amount of extra time. Moreover, some plasma is still present between RBCs settled at the bottom because they are settled by force of gravity only. Hence, a lot of plasma is lost as well.
      On the other hand, centrifugation saves time. And separates RBCs with force greater than gravity. Hence, no plasma is lost in the process.

      @@metinali1691 show the area to your local Dermatologist (Skin Specialist) first. He/she will evaluate whether hair can grow back there or not. For example, if the area has scar with fibrosis, hair cannot grow there.

      I don't think taking blood will have any benefit. But taking plasma will surely have benefit because growth factors, etc are present in platelets and plasma and not inside red blood cells. Yes, both poor and rich plasma is taken and blood is discarded in the video.

      Yes. They cannot be distinguished as separate layers. It is just known that plasma near the bottom is rich in platelets.

      No side effect. But the effect of treatment only lasts as long as you continue doing PRP. Initially you do it every month. Then once every six months.

    1. I Have a old centrifuge Machine it's not digital there's a dial on it which refers to speed it's mentioned from 5 to 40 . . So i want to know the settings at which speed scale or time should i set. .
    2. Can i do prp in standard cbc vials of sodium citrate? If Yes kindly tell me the best settings of centrifuge according to these standard cbc vials & centrifuge machine which i have mentioned above in my 1st question

      1. Settings should be 17 in Round 1 and 22 in Round 2 as mentioned in the video.
      2. CBC vials have usually EDTA in it. It is better to use vials having Sodium Citrate inside. Use the vial on which it is mentioned that it has Sodium Citrate inside.
      3. No need to add anything in the PRP. Besides PRP, use 5% minoxidil hair spray daily 3 to 4 times and take vitamins orally regularly. Hair spray should not be used for 24 hours after PRP. After that, continue using it again.

      When they pass through low gauge prp needle, they get activated. It is desired that they get activated at that time and not before that. Therefore, prp syringes should not be tapped or shaked to remove air bubbles until the time of injection comes. Air bubbles should be removed just before injecting the prp.

      Blue Vial = Sodium Citrate Vial or PT/APTT vial. Because PT and APTT Tests are also done in it.
      Red Vial = Serum Vial. It has no chemical inside.

      Blue contains anticoagulant (Sodium Citrate). Red is called Serum Vial. It has no chemical in it.

      @@nonamemyname862 Using a tube (vial) that contains some anticoagulant is necessary. Usually Sodium Citrate containing vials are used. It prevents blood from clotting and the separation and whole process is possible. Otherwise, blood will clot and all the RBCs and platelets will cluster up and the whole process becomes useless and impossible.

      That "Red Spot" at the bottom of the vial after 2nd spin is actually RBCs (which were previously dissolved in the plasma). They are discarded.
      Note: A single spin is also enough. Two spins are better though.

      Proper technique must be followed. Otherwise, local anesthesia effect will be lower. Keeping pressure for a few minutes on the lower part of eyelid is important, otherwise, the injected medicine will diffuse to other areas and effect will be lower.

      Red Top Tubes are called Serum Vials. They do not have any chemical or anticoagulant inside them. They are just empty vials.

      Different types of vials are made for carrying out different types of blood tests. Some have EDTA as anti-coagulant in them. Some have Sodium Citrate as anticoagulant in them. Some have no anti-coagulant at all, etc.
      For PRP, it is recommended to use vials having Sodium Citrate as anti-coagulant in them. These vials are usually used for carrying out blood tests called PT, APTT & INR. Therefore, these vials are also called PT/APTT vials.

      Water vial is placed in the centrifuge machine just for the purpose of maintaing balance in the machine, because the machine cannot rotate a single vial properly.

    So the growth factors will regrow hair on the scalp? But i guess the follicles need to still be there for it to work, right?
    On another note I've watch other video where they centrifuge once at 3500 and platelets show above the gel. They discard most plasma at the top, which they call PPP (platelet poor plasma), and then collect just the remaining with the platelets (PRP). It seems it may be more concentrated this way.

      Yes, you are right. Hair follicles must be present for hair to grow. Therefore, dermatologist must be consulted before starting the procedure.
      Yes, some people discard the top platelet-poor plasma. However, I think, it is best to just use both the plasmas. Because doing so will increase the total number of platelets injected into the scalp. On the other hand, if the procedure is required in some other body area or joint where only limited amount of plasma fluid should be injected, chosing only PRP and discarding PPP seems better. But for scalp, using both is better.

    when i went to my hair clinic, before starting PRP for Hair. They told me to get tested for Hiv, other STDs,..
    I want to know is this procedure safe? Because I see a lot of blood work and injections.
    Will STDS get transferred if clinic re-uses syringe, needle of infected person who came before me for PRP. Is there any risk?

      Yes. STDs can be transmitted from one person to another by reusing syringes, etc. However, if proper precautions are taken, it is not transmitted. Most important precaution is to always use new syringes for every new patient.

    Use gloves for plasma separations etc.
    Never use a Potassium Citrate Vial as it contaminates the PRP
    When you have to collect all the plasma even after the 2nd round of centrifugation.. then what’s the purpose of 2nd round of centrifugation?
    And in my life I have done a lot of them, no patient chooses to do it without anesthesia as it is painful.
    Never do it with an incomplete supraorbital block.

      Sodium Citrate vial is used. Not Potassium Citrate vial.
      2nd round separates residual Red Blood Cells. However, this step can be skipped. It is not compulsory.

      @@MedTimewithKhan Yes sodium citrate vial should also not be used, plus the second round does not separates only residual RBC’s.
      Even if there is some residual rbcs in PRP, there is no reason to separate it.
      We cannot skip the second round as it has it’s own significance it divides the plasma into platelet rich and platelet poor plasma. Go for platelet rich plasma which is settled in the base.
      Ultimately people get results.
      With this method I am sure the patient would not have a very satisfactory results.

      You can buy it from any local medical store/chemist. Just ask him to give you LP (Lumbar Puncture) Needle. Size = 16G or 18G.
      Note: This is the same needle used for giving Spinal Anesthesia to a women before C-Section. In that case, anesthetic medicine is injected into her lower back through this needle.

      Initially one session per month for 3 to 4 months. Then one session in 3 months. Then one session in six months, etc.
      Pyodine is usually not required before injecting PRP into the scalp.

      @@abbubakar7444 5% Topical Minoxidil Sprays and oral multi-vitamins, etc should be continued regardless of whether PRP is being done or not. PRP is an additional therapy to further help these.
      Note: Just skip Minoxidil therapy for 24 hours after every PRP session. After 24 hours, continue Minoxidil therapy normally.

      PRP slows down the process of hair loss. It cannot stop it completely. It also cannot regrow hair that are already lost.

      I think your blood sample was not properly mixed with the anticoagulant in the beginning because of some reason. Follow these points for avoiding it in the future:
      1. Buy the vials from some good quality medical store. Make sure you can see anticoagulant drops in the vial before using it.
      2. Put Blood Sample in Sodium Citrate Vial as soon as you draw it. A delay of even a few minutes is not good because blood would have started clotting already.
      3. Fill the vial up to the top mark. Do not fill above it. Otherwise, anticoagulant will not be sufficient for the given quantity of blood sample.
      4. After filling the vial, gently roll the vial in hands so that anticoagulant is properly mixed with blood.
      5. Now spin. Even a single spin is sufficient. But blood and plasma should be properly separated and there should be no clotting.
      If above points are not followed properly, blood can clot and whole process will be failed.
