At the start off the boss fight when I was watching a theory that MatPat made, he said "Where... fighting ourselves."
"I don't think that's a clone, that has to be Peppinos brother." I thought. And at the end of the boss fight where it's the chase scene, it's like he's chasing Peppino out of his room, like how a bigger brother does to there young brother when there annoying them. Next, is gonna be how Fake Peppino looks like how he is.

Fake Peppino after he moved out from his parents house, he built a Pizzeria, he named it "Bruno's Pizza" he wanted Peppino to be a chef, but he was still in High School, Fake Peppino made a couple off orders, and then when his oven broke, it caught the Pizzeria on fire, it melted his skin, he later woke up with burn marks around him and his skin melty, so he went around town finding a job. Pizza Head gave him a job and teached him how to cook a pie. When Fake Peppino was asleep, he grabbed a piece of his melted skin and made a clone of him.

When Fake Peppino and Peppino see each other, I imagine them having a conversation with each other. Here is what I think there saying secretly. "Yeauuuuuu!" They yell. "Wait... Brother?" Fake Peppino said. "What, how are you here?" Peppino asks. "I signed up for a job here to be a pie maker." Fake Peppino says. "IT'S NOT SAFE HERE! THERE ARE BOMBS, BOSSES, AND MORE HORRIBLE STUFF!" Peppino yells. "What? Stop making up lies you idiot." Fake Peppino said. "Just-a leave." Fake Peppino said. "NO! YOUR GOING WITH ME CAUSE THIS TOWER IS GONNA CRUMBLE AFTER I'M DONE WITH PIZZA FACE!" Peppino screamed. "NO! You probably caused that fire at my Pizzeria!" Fake Peppino yelled, then Peppino gets angry and hits Fake Peppino with a Chair. "You want to fight? LET'S FIGHT!" Fake Peppino screamed. Then the fight happened. Then the scene where Peppino gets chased by Fake Peppino out of his Pizzeria. "OH GOD! OH GOD!" Peppino screamed. "I HATE YOU BROTHER, BETRAYING MY TRUST!" Fake Peppino screamed. Peppino gets out and tells Fake Peppino he's sorry "Sorry brother, but you would see after I'm done with Pizza Face, bye." Peppino runs to the elevator to head to the last floor.

Based on what I’ve researched from TvTropes, Fake Peppino used to be a human named Bruno who runs his own pizzeria, but he got abducted by Pizzahead and mutilated him into a deformed clone of Peppino; due to his almost identical appearance.
Probably at the end of the game, Peppino “adopts” Bruno | Fake Peppino as his younger brother and allows him to help out in the
kitchen out of kindness and consideration.
It may offend his estranged brother Maurice with the thought of being replaced by a molten-looking skinwalker, but it won’t matter to him.
