Thanks. I have a client requiring me to get my own workers comp but i don't want to get it since I'm a one-man LLC. Is this the same waiver I need and does it need to be issued from my state?

When you say a "Template" i'm not sure what you mean? There is a standard form that your insurance agent is going to have to produce for you. If you don't have an agent, we'd be happy to work with you!
