- Heparin is an anticoagulant (blood thinner) that stimulates antithrombin III (ATIII) which then inhibits coagulation factors VII, IX, X, XI, XII, as well as thrombin. This leads to prevention of thrombus formation (anticoagulation)...Heparin is used for ICU anticoagulation...Adverse effects of heparin includes bleeding, Heparin-induced thrombocytopenia (HIT), and osteoporosis.

Wait! but as far as i remember from ur lectures, aPTT was for both intrinsic and comn pathway and PT was for extrinsic and comn ...
So how does LMWH doesn't affect the comn pathway prolonging both aPTT

For monitoring UFH you can use the UFH assay and aptt?
At 7:00 min mark you mention that LMWH does not affect ptt, but on clot based measuring assays ptt would be affected (increased) because factor X is needed to form a clot. I'm confused in this aspect. A response would be great help.

hello legendar videos as usual . what confuses me is that i was taught in university that heparin potentiates the effect of antithrombin III on factor x and ii which are in the common pathway, so why does heparin increased ptt
Because in reality heparin activates antithrombin III which inactivates factors II, VII, IX, X, XI, and XIIâŠThatâs why heparin raises PTT.
If heparin is in the body all the time, why can the cascade lead to hemostasis? Wouldn't antithrombin just prohibit all clotting before fibrinogen could be activated and made into fibrin?
Your body has both antithrombin and the clotting factorsâŠThe balance between the two is what keeps you in homeostasisâŠIf antithrombin wins, we bleedâŠIf clotting factors win, we make a clot. Obviously, we want to form a clot to stop bleeding when we have a cut (homeostasis), but we donât want a heart attack or a stroke (pathology: disturbance of homeostasis).

Thatâs true, because PTT is measured until the thrombus is formed, and there will be a delay of thrombus formation with LMWH or Fondaparinux...I was oversimplifying...I shouldâve bean more specific!

@@MedicosisPerfectionalis Your welcome :) . Even warfarin ( among other drugs) can affect aPTT . The whole discussion is important when you order an aPTT and then start looking for answers why it is slightly elevated.
