Most of the danger is in moving from island to island, especially since the magnetic field makes it nearly impossible to find your way back home if you leave, plus the crazy weather and sea beasts. So countries can form and keep to themselves, but trying to explore multiple islands is where it gets tricky.
But honestly, I remember reading the manga as a child when it was first coming out in America and just imagining the Grand Line as, like, a giant version of Tortuga from Pirates of the Caribbean. I was calling out my younger self as much as I was calling out you. It was an understandable assumption to make and Oda had a lot of fun pulling back the curtain and showing us just how big and complex his world could get.
