Often accidents have more the one contributing cause as demonstrated in this case. Inadequate toughness of materials specially at low temperatures, upset process conditions, poor mitigation measures. We can never eliminate failures, but we can minimize them and lessen their impact. Good report.
The reported corrosion average of "20 to 30mm per year" is an error that should be corrected.

"PIG" does not stand for either "Pipeline Inspection Gauge" or "Pipeline Intervention Gadget" or any other such nonsense - they just look a bit like pigs, and the original ones were made of straw bals wrapped with barbed wire which made a loud squealing noise.
