Helios Airways Flight 522 | The 737 Doomed by the Flick of a Switch
In the early morning of August 14, 2005, a Helios Airways Boeing 737 lands at Larnaca International Airport on a routine flight from London. The crew noticed abnormal noises coming from the right aft door and so maintenance carry out a full overnight inspection. The check involves what is known as a pressurization leak check - which requires setting the pressurization system from the “auto” setting to the “manual” setting. The inspection is completed but the ground engineer forgets to switch the system back to “auto”.
The following morning, the crew prepare to depart on a charter flight to Prague via Athens. On board are Captain Hans-Jürgen Merten, first officer Pampos Charalambous, 4 cabin crew members and 115 passengers. The pilots perform their routine pre-flight procedures, but miss the pressurization system switch which is still in the wrong position. Subsequent checks are completed as per procedure, and the aircraft takes off just after 9am.
The aircraft begins a steady climb to their assigned altitude of 34,000 ft. As it passes 12,000 ft, the cabin altitude warning horn sounds. This confuses the pilots as the horn has the same sound as the take-off configuration warning. Shortly after, at 16,000 feet, the captain contacts the company’s Operations Centre and reports that the take-off configuration warning light is on. Before speaking to an engineer, the aircraft passes 18,000 feet, where the cabin oxygen masks are automatically deployed. The captain finally manages to speak to an engineer, and repeatedly states that the “cooling ventilation fan lights are off”. The engineer asks the captain to confirm the pressurization panel is set to AUTO. The captain begins to display symptoms of oxygen deprivation, known as hypoxia, and disregards the engineer’s question and instead asks “Where are my equipment cooling circuit breakers?”. From that moment on, it was radio silence.
The aircraft, with an unconscious flight crew, continues the climb to 34,000 feet. At 09:37, the aircraft leaves the Nicosia FIR and enters the Athens FIR. Several unsuccessful attempts are made to contact the aircraft. At 10:21, the aircraft, following its programmed route, overflies the approach’s main navigational fix, the KEA VOR. It then proceeds to overfly Athens International Airport and returns to the VOR and automatically enters a holding pattern.
Two F-16’s are dispatched to intercept the aircraft in order to establish visual contact. They report that no one is in the captain’s seat and the first officer is motionless. During the 10th holding pattern, one of the F-16 pilots observes a person entering the cockpit, sitting down in the Captain’s seat and putting on a set of headphones. This person is flight attendant Andreas Prodromou, who is miraculously still conscious due to the use of portable oxygen bottles available to flight attendants. Shortly after, with the aircraft running out of fuel, the left engine flames out, forcing the aircraft into a descending left turn. 5 minutes later, Andreas, who is struggling to remain conscious, sends out a series of distress calls to Athens ATC. These calls are never heard, as the radios are still tuned to Nicosia ACC (area control center), who at this stage are well out of range. It is only at 7000 feet when Andreas finally acknowledges the F-16s escorting the aircraft. At 11:59, the right engine also flames out due to fuel starvation. The 737 becomes a glider and continues to descend rapidly. Despite Andreas’ best efforts, the aircraft crashes into the hillside near Grammatiko Village, 33 km northwest of Athens International Airport.

    So the man who was ultimately responsible for the crash, the captain of the KLM flight, is one of the few who survived. What a tragedy all around.

    I'm from Tenerife, here is a gross fact: Many people reported the smell of burning and rotting flesh for DAYS in zones near the airport, also, some pieces of it also went to houses near it too...

    Even before the crash, the layout of the airport looked like a disaster waiting to happen. That many planes should not have been on a runway that looked like it couldn't even accomadate TWO large planes safely.

    I have no experience but I believe that if there is low visibility, then you should halt the take offs, especially if there is extremely poor weather and nobody can see eachother!

    I was there. I live in Etobicoke. I know these conditions and this runway. I saw the result. The pilots screwed the pooch. Thank God the cabin crew chose an uncommanded evacuation. All out in under 2 minutes. Brave crew. Stupid commanders.

    I am a Maritime professional and I did a course called Maritime Resource Management where the course structure was based on this particular incident. The root cause was miscommunication and misuse of authority.

    My late grandmother saw this happen from the airport terminal. It was a terrible tragedy and I was truly grateful that it wasn’t her on either plane.

    The KLM captain gets a lot of the blame for his recklessness and rightly so, but in my opinion, not enough blame was laid at the feet of ATC who really did a poor job in ensuring Pan AM was off the runway and this was clearly communicated with KLM despite the radio interference. I heard many of the ATC staff were watching a soccer match during this series of events and given the high stakes and poor weather, this was incredibly negligent. Also words like ‘cleared’ shouldn’t have been used when take off clearance hadn’t actually been issued.

    You fail to mention a very key point which were the strict Dutch aviation regulations around how long pilots could be on duty. Had KLM not taken off, the pilots would have been heavily penalised so there was pressure to take off sooner.

    Insane. Learing that KLM shouldnt even have been on Teneriffe but on a whole other island, makes me sick. The amount of bad luck needed for this catastrophe to happen is insane.

    Tenerife airport disaster, the deadliest aviation accident on history, with 583 deaths 61 survivors 60 by pan am, 1 by KLM, who stayed on the islands with her two freinds instead of reboarding with her two freinds, this day flights should not fly in foggy days, miss-communication was the cause of the crash, KLM took of without a clearance, two B747 crashed, very scary.
