His sanity could be a ghoul thing, like some kind of perfectly balanced neurotransmitters and protected neuron sheaths, if their cells regenerate perfectly and don't lose the telomeres that regulate ageing and DNA has to protect itself during replication (but eventually cells run out of it, as the telomeres aren't replicated, just the DNA sequence). It's an interesting concept for an encounter, but it being so casual and the lacklustre execution, despite the should-be horror of the situation, is a letdown. Experiments into total darkness on real humans lead to total loss of perception of time, unusual thoughts, then hallucinations and such. Usually, experiments are ended within a few days at most.

His parents claim they've been looking for him (for 200 years) but the fridge was like 2 mins away from their house? And AS SOON as you help him, you get dragged into a conflict with the Gunners that were harassing them. It all seems **too** convenient. Like they were banking on a good samaritan to deal with the gunners for them. But then again, Fallout 4 isn't really known for stellar writing. So yeah...

Honestly though I question if anyone would be able to remain isolated in a fridge for 200 years not having any contact with anyone and not go completely insane.

I think cait knows theres no way ghoul or not billy could have been in that fridge for 200 years. Even if ghouls dont need food or even oxygen to live anymore how did he not go feral or even crazy from 200 years of both isolation and sensory deprivation? Humans start to lose it it sensory deprivation after as little as a few day but 200 years? She probably thinks its some con the kid pulls every now and then. See someone coming from a distance, ducks into the fridge when hes them coming sure theyll hear him. Asks to be let out and then have whoever gets him out take him home to his parents who act like they havent seen their son since the day the bombs fell.
