This is the one film I've seen where they actually touch up on the question of one's own internal existence rather than the external. I'm not talking virtual reality, but the idea that you yourself are real. Or at least, real in the way you believe. David does ask the therapist a very good question, how do you know? You certainly feel real, but you can feel a lot of things that may not be real. You technically don't have any hard evidence you exist, just your senses and thoughts telling you that you exist.
