This isn't a bug. Don't walk into the physical bell, that's whats caused you to die here. I generally wouldn't recommend activating them with a melee for this reason. The area in which a player is teleported after the bell is activated is shaped like a donut around the bell itself. By hugging the bell you are standing in the hole of the donut and thus aren't teleported, and if you aren't teleported you die a second or two later. Activate it then take a step back into the ring of the donut and you'll be fine. Generally, anything that is clipping into an object outside of the teleport area isn't sent to the other side. You can verify this by going to the lower bell on the dream (blue) side then backing up in the dividing wall between the lower and upper bells before shooting the lower bell to activate it. You'll actually get left behind and won't be teleported to the nightmare (red) side for several seconds.
