This was a 10/10 Laser chat filled with some wonderful pearls. No one talks about using the 515 filter for treating hemosiderin staining and for dealing with select cases of PIH. This is the 1st time hearing anyone mention that drizzling lidocaine once the epidermis is removed aids in the transdermal delivery of anesthesia into the papillary and reticular derrmis. I suspect I am a bit of a loner in so far as I use epinephrine for hemostasis when performing full field ablation. I do this because I feel ablating tissue that is bleeding: 1.causes a mess 2. so much of the energy is absorbed by the water in the blood that we no longer can reliably determine at what depth we are. I therefore stop the ablation by depth and not the appearance of pinpoint bleeding. I do so because I fear that a change in bleeding patterns is too subjective (in my hands), and it's such a critically important endpoint. I totally agree with Dr. Siperstein in that profractional on top of full-field ablation creates for a more even "healing process" with less "striping". In the patient who was in the wheelchair, I was unsure if the patient actually had 2 passes at 200 microns of full field ablation under the eye or if the total depth was 200 microns. Finally if one uses the appearance of bleeding patterns to determine that we are in the mid reticular dermis causing up to stop then what effect does adding coagulation have on blunting the "bleeding pattern endpoints". These sub -ablative pulses are likely coagulating blood vessels which to me might cause the reliability of bleeding patterns to become less reliable. These are just "points to ponder" as I think through all of this technology to reverse visible signs of aging while trying to maintain a zero complication rate. Nevertheless I feel grateful to have heard this Laser Chat with Drs. Sipertein & Brady, and I feel they deserve an encore!

I performed 1470nm 400 20% and 2940nm 20 18% on skin type 4 Asian pt w melasma. It’s been 1 week but her melasma got worse. I don’t think going past 15% on someone with melasma is a good idea.

Do you have store in india? Wanna purchase this machine..

On the halo sciton protocol it says 1470nm depth for skin type 4 (Asian) should be 350-400um and Dr. Chris Robb’s melasma setting for type 4 is 400-500um for depth. So is doing 325um ok to do for melasma pts who is skin type 4? Thank you

I’m having Sciton deep resurfacing in a few months. The reason was primarily for tone and texture. The PS suggested that I didn’t need the whole face done but only the peri orbital and around the mouth, marionette lines. After watching this it makes me wonder if he is only addressing the wrinkles which wasn’t my primary concern. How can you address tone and texture in just those areas when clearly my whole face is in need. Did I not ask the right questions? Very well respected surgeon who has done other procedures on me with excellent results so I trust him I’m just wondering if we were speaking the same language. The down time is 14 days and my understanding is he is doing the strongest he can do. And it was expensive compared to other estimates. That being said he is an excellent surgeon. Help! 🤦🏻‍♀️☺️

Very well done Erin, Tamara, & Dr. Bitter. Most of my patients could not tolerate 60 microns at 22% with the 2940 without 30 minutes of a strong topical. I would liked to know how you routinely control discomfort with ablative settings as above.

Should BBL be avoided during summer months? Or as long as you not sitting in direct sun and always have your SPF on it is okay? If its not okay, what Scitcon treatment would you suggest for overall maintenance. I've seen the clear silk is new, but I haven't seen much info/before & afters.
