Thank you for recording and showing the dynamic scanning and pointing out the anatomy along the way! I feel that many other resources simply show the ultrasound image or a brief video of needle driving which are not the things that are difficult in the fascia iliaca block. Target acquisition is the hardest part!

The site is within the thoracolumbar fascia, at the plane between the psoas major and qudratus lumborum muscles

I will quote from here: pubs.asahq.org/anesthesiology/article/130/2/322/20073/Quadratus-Lumborum-BlockAnatomical-Concepts "The proximity of quadratus lumborum block to the pleura and kidney in the subcostal anterior approach presents a risk because of direct needle trauma.10 The risks of bleeding complications are not yet known. There is no evidence for the stratification of the risk of bleeding based on quadratus lumborum approaches. The posterior and lateral quadratus lumborum block approaches use a fascial plane through which the abdominal branches of the lumbar arteries course. The anterior quadratus lumborum block is a deep block, close to the lumbar plexus and risks retroperitoneal spread of hematoma. Therefore, the authors recommend that, for all approaches, the American Society of Regional Anesthesia and Pain Medicine guidelines for deep peripheral blocks be followed." In summary, there are theoretical risks, but objectively there is no concrete evidence (yet) of increased complications with various QL blocks.

For CS, WHAT LEVEL WE GIVE WHEN DOING ESP BLOCK.T10 OR T11?
point of injection of whether T10 or T11 doesnt make much of a difference coz the injected volume spread will carry it there anyway.. at least at a sonographic point of 4-5 TP caudad away from the end of rhomboids shd be enough if intention is for post op incisional wound analgesia at T12- L1

Excelent work. I’ve seen other renowned doctors showing on presentations a rate of up to 75% of obturator nerve block with 40 ml of LA on the supra inguinal fascia iliaca block. I suspect live human tissues have different viscosity and lymph between muscles that may “dry” and “adhere”, promoting a more difficult spread of LA (my theory). A subsequent study using LA+ contrast could be made in patients pre hip surgery to enlighten us better and a MRI exam right before surgery (choosing elective patients). A clinical vs cadaver study made by the same blocker would be very interesting. This question is of pivotal importance in the pursue of hip arthoplasty analgesia improovement. Should we choose the supra inguinal FIB or should we aggregate the PENG block + FN + FlatCN block? Wich route…? 🤔

Intersting input... agree.. living and cadaveric tissues may provide different dynamics for fluid (ie LA) spread which may affect clinical presentation. Even when injections are done by the same operator, inter-individual patient variability may also affect clinical presentations observed. Ideally, in centres whereby the facility is available, contrast studies done immediately after injection is best to assess spread but unfortunately its not available in many centres and it is not practically possible due to logistic reasons. As for choice of block, those 2 are definite options, but to consider the need for coverage of incision site, my choice would be the FICB.. and via the suprainguinal approach

The whole things was good but 90 degree rotation never showed on the body but only on the screen, that was so so annoying. Please show on the body in some other demonstration.probe on the patient and 90 degree rotation was a joke. Sorry
