Dr. Trauer, thank you for your excellent teaching. I have a question - absence of lung sliding is, in my experience, not very specific to pneumothorax. It can occur in pneumonia and pleural effusion. Needle decompression is not without risks - my colleague hit the lung once and the patient bled out. The context is different in an arrest, but in a peri arrest I would not personally rely on absence of sliding.

I have some questions: where do you put the tip of the catheter and what is the suggested rate according to different numbers of the fractured ribs? How does the location of the fractures influence your decision of the injection site and direction of the needle / catheter? For example, today I had the patient with fractured ribs from 1 to 9 of the Rt side...

I learned so much on this video thanks a lot !! However i'm a bit confused about how should/could I recognise a tension pneumothorax when somebody is in cardiac arrest since he won't be breathing and there won't be any pleural sliding .. This is something i don't understand, if could explain it please ! Thxx :)

Thanks for this nice presentation. If we want to block T2( intercostal brachial n) , can we get the probe at higher level T3 and give less volume.
Pleasure Gamal! Sure, you can adjust the level as necessary depending which ribs are fractured. Can also tilt pt post procedure to encourage spread in a particular direction. I'd suggest always use at least 30-40mL to achieve adequate spread, just dilute as necessary
