After a thorough facial diagnosis, along with cephalometric diagnosis, we begin our treatment plan considering what is desirable and what is possible.
In cases of excessive vertical maxillary growth, for instance, what we can do within the limits of mandibular size and its sagital position is to impact and distalize the maxilla, allowing for mandibular autorotation.
However, this requires that the facial and cephalometric characteristics are suitable for such a procedure.

Moving the posteriors mesially doesnt mean we are loosing achorage right. It means we are using class b anchorage.
It depends on the terminology you use. The types of anchorage A, B, or C are terminologies usually employed in segmented arch mechanics. What I refer to here as anchorage loss is a terminology widely used in Brazil and other countries for this type of movement.

You don't need to extract, that will lead to a smaller mouth profile and possible complications in the future. SARPE or Jaw surgery would be addressing the bigger picture. This patient has a very narrow palate and should be expanded. It not that difficult to see. Its another question if the patient does not have the money or the courage to proceed with surgery.
@@ZonkedSardar It’s astonishing how the proponents of orthotropics and other pseudosciences always resort to the same arguments. ‘Never extract, don’t ever extract.’ But there are clear indications for extractions, just as there are for expansion and distalization. None of these decisions are made arbitrarily-they are grounded in pure science and precise indications. So, it’s pointless to approach me with the same rhetoric unless you back it up with real science. And by real science, I mean published studies in high-impact journals with well-conducted research. The truth is, extractions are sometimes necessary. In this case, expansion was entirely contraindicated, beyond other points, to avoid on the narrowing of buccal corridor.
