Difficult Airway Challenges
Welcome to another episode of Vital Times the CSA Podcast. I am your host, Rita Agarwal. For this episode we went back to the basics and talked about something that all anesthesiologists care deeply about - managing the challenging airway. Joining me is a former colleague and friend who is an attending anesthesiologist at one of the busiest trauma centers in the country - Denver Health Medical Center.
Dr. Alma Juels is an Associate Professor of Anesthesiology at the University of Colorado School of Medicine and the Director of Quality, Safety and Improvement, and the Director of OB Anesthesia at Denver Health Medical Center. She is also an Expert Consultant for the Colorado State Medical Board and has lectured extensively, presented workshops and cared for many patients with difficult airways. In this podcast, she will share some of her scarier and/or more interesting experiences.

    I guess, Cost effectiveness is the main motive. How can one consultant run four OTs with the help of AAAs or whatever they r called,while ASA itself mandates presence of qualified anesthesiologist with each patient at 1:1 ratio? Practical skills can be imparted to any person with considerable IQ but recommended theoretical base is vital to safe application of that skill on the "living" patients. You cannot train a cab driver to pilot a jet without completion of standard formal course.

    Lovely short about anesthesia and I'd like to make a point about medical students being responsible for ether since there were no Anesthesiologist in 1923. By 1923, there were 19 Nurse led anesthesia schools in the United States and Alice Magaw, a nurse anesthetist, was well on her way to completing over 14,000 anesthetic procedures without a single recorded death. In 1899, Magaw had already published a scientific article in the Northwestern Lancet about the role of nurse anesthetists at the Mayo Clinic. In addition, 1923 was the year when Agatha Hodgins, the nurse anesthetist who pioneered nitrous oxide anesthesia with surgeon George Crile, first suggested the creation of a national organization for nurse anesthetists at a meeting of the Alumnae Association of the Lakeside Hospital School of Anesthesia, established in 1915. Nurses have provided anesthesia since the Civil War. When discussing the history of Anesthesia, it is important that we recognize where we came from considering nurse anesthetists were the first healthcare providers dedicated to the specialty of Anesthesiology.

      Thank you for this thoughtful comment! Although some physicians like Crawford Long (1815-1878) administered surgical anesthesia well before the Civil War, it is true that non-physicians (i.e. nurses, medical students, even patients’ family members!), as well as interns and junior surgeons, predominated in the anesthesia workforce in the late 19th and early 20th centuries. This was the era when open-drop ether, a rudimentary anesthetic modality by today’s standards, was the standard of practice. Nurses were indeed prominent practitioners of anesthesia in the U.S. since the late 19th century. Alice Magaw (1860-1928) and Agatha Hodgins (1877-1945) are prime examples. Organized physician anesthesia in the U.S. began in 1905 with the creation of the Long Island Society of Anesthetists, the earliest precursor of the American Society of Anesthesiologists. But the first national physician anesthesia organization was the American Association of Anesthetists (AAA), founded in 1912 by physicians like James Gwathmey (1862-1944) and Francis McMechan (1879-1939). The video’s mention of the lack of an anesthesiologist in 1923 is not meant to imply that there was an absence of physicians administering anesthesia during that year, but rather to suggest that the numbers were still low. As increasing numbers of physicians specialized in anesthesiology, they began to pioneer scientific advancements that modernized anesthetic practice and transformed it into a medical specialty. The history of anesthesia and anesthesiology in the U.S. is a rich and nuanced one, and both physicians and nurses have contributed to it in different ways.

      The AANA (nurse anesthetists) has advocated to use the word “nurse anesthesiologist” so that they can call themselves anesthesiologists, which confuses patients regarding their level of training and expertise. So now we use physician anesthesiologist to indicate we are medical doctors with a different degree of training.

      @@sburt1998 There is simply no comparison between the number of hours and level of experience provided by an anesthesiology residency vs the two years of CRNA training. CRNAs play an important role as part of the anesthesiology team, but it’s unfair to patients to think they are speaking to a board certified medical doctor when they are not.

    They are only allowed to work under the strict direction of an anesthesiologist and only in a hand full of states. I really feel bad for them.

      Why would you feel bad for them? They make the same money and do the same job as a CRNA would. Sounds like a good gig to me. Though, It does suck that they have to fight the nurse lobby to get established in the rest of the states.

      Guys just STOOP FEELING BAD about any faculty especially when you know nothing plz stop being annoying plz it,s enough . CAA we are the Wright hand of the doctor anesthesia is a *team * and doctor's literally can do nothing without us we need each other bruh and we love that , don't be sad cause with experience for years we can do a lot of thing u don't expect from us to do so again don't feel bad ! Actually we are applying most of the thing that we have being told and some of are dangerous. We do many thingx very important and we know better than doctor to performing it , 6 year studying just anesthesia not like 2 or 4 year time we spend on this will not gone like nothing , not waitng u to say shit actually. Finally i believe in us and education and alot of these year on it are something valuable slso our experience and someday we wil be appreciated.       because doctors work 80 + hours a week. have to go to school/train for 12+ years , residencies are 100+ a week . AND on top of that take out extreme amounts of student loans (150k-350k) and they're getting less and less money .
