i see many patient have this procedure done plus post and cord and crown done, and it snap off at the gum line after several months, all the money total $4000 wasted. please advice patient to have it extracted and place implant there. the cost is cheaper. stop doing this to the patient. dont try to be a hero.

This comment section is clueless: First of all, this case is as of today 8 years old. Much have changed since that time, but ''old school'' prosthodontics stil works when restoring function, and in some cases aesthetics. Newer materials have better aesthetics, but they lack evidence compared to PFM, especially regarding longevity! Significantly worn down anterior dentition need strong durable material when restored. PFM crowns with occlusal metal provides this, and has support in literature i.e. we KNOW this will work. Could probably been done with zirconia for better aesthetics. However, this might have been out of the patients price range as zirconia is more expensive. Other ceramic materials I would not recommend due to their tendency to cause tooth wear --> which is a major problem for this patient!. Regarding aesthetic: Low lip line / Smile line covers metal margins as shown in photos, i.e. not visible!! Note: Covered metal margins for PFM makes for less hygienic condition in cervical portion of teeth. Nevertheless: The aesthetic is improved compared to the pre-treatment photos. Surgical crown lengthening: To save tooth structure, increase pilar height, thus making proper retention more achievable, and lessen the extent of OVD increase. The OVD is increased in this case hence the posterior open bite, which will even out with time --> Please look in to DAHL CONCEPT. This concept should be well known for all dentist. Sadly not the case in this hubris comment section.
