    Hi Dr Rege, yesterday I received a BPD diagnosis from my psychiatrist (as well as C-PTSD + my previous OCD & PMDD). I’ve been suffering for a very long time and one bothersome thing to note is a psychiatrist I had put “possible BPD” in my chart 3 years ago but I was never told that he did that until yesterday. I admit that I may not have taken that info well at the time but it’s been a lot more empowering to know in the long run. I wish someone would have just told me years ago. My social worker was the one who separately brought the possibility up to me 6 months ago and since then so much has clicked as I’ve been able to learn about myself and dive into the wealth of info available online + it’s been nice to just know there’s other people who are like me. I even watched this exact vid beforehand but had no idea BPD could apply to me. My psych has offered me Lamictal or Abilify to think about and decide what I’d like to take. Do you have links for info about comparisons, efficacy etc. I’m already on fluoxetine, LDN and valium which especially helps the agitation/anxiety. Just looking for help knowing what I should proceed with as there’s not much time to chat at psych appts. I’ve also heard Dr. Lois Choi Kain, a prominent BPD advocate speak about a recent study on Lamictal for BPD which says it had no greater effect than placebo… not sure if you’ve heard this?

    While methamphetamine use was fairly common in Germany at the time, and Hitler may have occasionally used it, I think it's a bit of a stretch to say it was responsible for Nazi aggression, and even more so the Holocaust. Political, social, and historical factors were far more important reasons behind Germany's (and Hitler's) actions in the war.

    Unfortunately, in our post-communist, now pro-Western fascist country, the Czech Republic. (The obedient dog of the western master). This drug is not approved. Hence, no one is getting this treatment here. But even if it does. I have seen the measly price at which the fascist pharmaceutical companies are selling this drug. :D I don't know exactly how much, but I know it was more than $1000 for a pack of 50 tablets. Blatant hyenism. The company should stand trial, or the government that allows such a company to exist. Even $20 would be too much. Even for $20, the company would have a royal profit. Producing meth is a primitive affair. Quick and simple, therefore at a very low cost. In addition, if you read the discussions about this drug, you will quickly realize that it is a very effective drug. Effective from a low dose, for a long time. I take this medicine, for adhd, self medicating. I have been using it for 20 years. It set me free from the first moment. My adhd was suppressed by the very inappropriate behavior of those around me. Using weed got me to the point where I was just smoking weed and sleeping. I was so extremely tired since I was little. No one wanted to understand me. They say everyone is tired... (Interesting that on the bus to and from school, I was always the only one who slept the whole way, I slept at school too). The vast majority of psychiatric medication is so drastic and devastating that I cannot understand how such brutal poisons can be so widely prescribed. Zyprexa, citalec, mirtazapine, rivotril, quietiapine. Adverse effects occur in almost everyone. The fact that, after a while, it seems as if they disappear is a huge self-delusion. Just because some of the side effects go away doesn't mean it doesn't continue to damage you drastically. I know cases who took these poisons for a long time. Unbelievable, they all have extreme problems. Damaged brain and entire physical health.

    Just wanted to add a comment here. I spent several months feeling exhausted and thnking I had CFS. I had a blood test and, much to mys urprise was diagnosed with Prediabetes.

      Prediabetes, In females conditions like PCOS - are inflammatory states associated with insulin resistance - they are part of the CFS picture very commonly. CFS is a syndrome with multiple domains - one of those domains is neuroendocrine ( prediabetes, insulin resistance) - it may not indicate a sufficient or necessary cause.

    Great hype toward benefitting Big Pharma. There’s, for one, there’s not a single record of any good outcome from use of psych meds. Secondly; long term use of prescribed methamphetamine cause brain damage.

      Yes. For Kernberg, narcissistic - NPDs are a subtype of borderline personality disorders. Pathological narcissism can only occur after the development of a self in which good and bad object representations have become integrated into a unified self-concept, whereas borderline personalities develop their pathology at a developmental stage prior to the development of stable ego boundaries.

    I am 54 was diagnosed with acute ADHD at the age of 11 and was prescribed with something that all I remember was it was beneficial and worked only problem was i was rarely given my doses as adults seemed to be taking it .the container was usually always empty even just a week after it was filled.. As an adult now I see that most cannot gain access so are forced to use street drugs and proper dosing does not occur. And the stigma surrounding this is as bad as the epidemic itself. Thank you for your video and your time and energy researching for this topic.

      I'm not even sure how it would be possible to get a legal prescription with appropriate dosing for success when all I have known is self medicating . There were times when I must have been close because I was happy reliable responsible holding a job and leading a decent life as a productive member of my community. Because of the establishment labeling program I am just a "meth head" I'd bet they are responsible for a good percentage of the illegal drugs on streets today. Once again thank you for your time

    You're forgetting the sexual side effects. When I was on Brexpiprazole I had uncontrollable sexual desires. Which led me to getting scammed out of $2,000 All I could think was sex. With constant erections. And with the long half life it was not fun. So what's receptor caused that doc I've been bipolar 2 depression for 25 years. The past 5 years treatment resistant until I found trintellix 4 months ago. And got diagnosed ADHD this week. So will ADHD medicines do the same thing as this medicine did to me?...

      Im sorry to hear! Yes its a side effect that occurs a lot more with Ari - what dose was it? There are warnings of Ari for pathological gambling now. This occurs due to the partial DA agonist effect - its interesting its happened with Brex as it is > antagonist - wonder if the dose was low? Thanks for the warning. With stimulants yes one should be cautious if this has happened with a dopamine partial agonist - please let ur prescriber know - as it may need cover with mood stabiliser - ps not advice. Having said that brexpiprazole can do this in the context of excessive D2 antagonism which leads to excess D1 phasic dopamine ( impulse dyscontol disorders ) - one can only know after an examination and history. Ps not advice

    I am also curious Dr. Rege, when I try and force positive emotions and feelings it's like someone suddenly puts on the brakes and either steers me into a ditch (negativity) or slows me back to a slow pace (flattening) , it's like my subconscious will simply not allow anything positive (or significantly negative) to be triggered?, the sheer power of something able to so easily override free thought is astounding to me!. This power includes sleep, where dreams are always kept flat and very boring!. I can see clear parity with emotional blunting and sexual dysfunction because of the non-reaction to stimulation, even my historically arousing porn does nothing (pure research!), I am also incapable of holding an erotic thought including dreams for more than a few seconds? Genital numbing is cruel reminder!. Can all this be attributed to 14 years of taking 100mg of an SRRI?.

      SSRI can be associated with this yes but of course other aspects need to be ruled out. Over time for some dose modulation is needed. In clinical practice in the context of such side effects a gradual reduction often can reduce or disinhinit the blunting ( like lifting the leg of the brake a bit - not completely ) .

      @@PsychiatrySimplified Yes I should have had a 'maintenance dose' at some point apparently and yes the effects of the original problem (depression/MDD) will have contributed towards my mental state as well my personal circumstances. I realise the many cases of SSRI side effects can be diverse and people can be affected to different degrees, I'm sorry if I seemed a bit 'full on' but I haven't been able to share this with anyone who understands before and express myself to this extent. I see two paths ahead, either gradual reduction of dose or change to Vortioxetine to better moderate the control of Norepinephrine, Dopamine etc as perfectly illustrated in your video. Thanks again.

    As a person cumulating both the rigidity and perfectionnism associated with autism and the constant reward seeking state of ADHD, this resonates deeply. Finding the balance between 2 seemingly opposite traits is something I struggle tremendously with. Thank you for that video. I keep hoping to find the magic answer even though there isn't such a thing. I feel like a puppet.

    Can you send me the diagram of the different neurotransmitters that ends in the dopamine production and if possible the inhibitory dopamanergic pathway (the VMPFC, DLPFC, and Insula-amygdala pathways if I am not wrong)!

    I I think artha and karma are explained nicely. But the golal of these aspects is gaing kama. . And the peace that can be attained by controlling "kama" (which may have a nice translation of getting pleasure) and the perseverance to achieve the pleasure slowly convert (this seeking or/and achieving of pleasure) to the state that reduces the immense importance of the kama, which is vairgya. From here, one can choose either the path of self-realization (considered as eternal happiness or moksha ) or using "vairaagya" to get the continuum of peace in this "laugika jeevita" . If we can translate these aspects into neuroscitific language, it may give some idea of the total activity of a yogi or a gruhasta. I feel so. Since the peace, the consistency of all happiness has the balance of kama and vairagya for a laugikaperson.

    L-Tyrosine supplementation would increase the raw material for the body to produce dopamine. If this is the choke point in dopamine production it may encourage increased dopamine only in deficient regions and not just randomly decrease reuptake like a dopamine reuptake inhibitor.

      In the context of stability yes- but in the context of other stress / poor sleep / oxidative stress etc modulation is what is needed to achieve homeostasis. Iron is a cofactor for tyrosine hydrolyse. Dopamine is 80% of the brains catecholamine content - dopamine neurons may be damaged in context of inflammation. Et

      @@PsychiatrySimplified good to know. my last blood donation showed a hemoglobin level of 174 g/L without supplementation of iron. I'll keep an eye on the other cofactors. recent supplementation of L-Tyrosine has really helped.. I normally have a low carb not quite keto diet yet I found having a small amount of carbohydrate just prior to bed improved my sleep quality. does glucose availability play a role in Dopamine production as well?

    Thank you, Dr. Rege. I am curious about excess dopamine and schizophrenia hypothesis I read about. I also read some other articles on scizophrenia and ph imbalance in the blood (1930) the blood was slightly acidic. Another, correlation or association was around half of patients with schizophrenia haf toxoplasmosis ghondi. Finally, I found another clue the calcium channel gateways in the cells. Clearly, there is something biochemical going on in this illness that is so complex. Can you discuss this? Elyn Saks, an attorney and Ph.D. wrote a book and did a Ted Talk about her turning point and diagnosis. I would love to hear about the scizophrenic illness and if it can go into remission etc.

    hey Dr. Rege, are you planning on doing a similar video on glutamate (and receptors) and the proposed mecanisms of actions as an antidepressant, as an alternative to SSRIs? 🤔 also, do you know if any such compounds exited human trials and have been proven more effective than SSRI/SNRI for specific conditions?

    Another important note is that the neurotoxicity is furthered by the hazardous compounds that go into its manufacture. Many of these can cause neurotoxicity as well.

    I have a prescription, issued by the Countys top court approved psychiatrist. I have enjoyed a successful career as a licensed contractor for insurance carriers, and there is truth to medical aspects of it.

      @PsychiatrySimplified I have a lot of useful information to share actually haha, Dr. William C Klindt and Dr. Daniel Amen ran brain spect imaging back in 2012, on my sincere quest to understand why my ADHD would lead to depression, addiction issues, etc. And it's a very controversial subject as you know, to argue that in order to successfully manage your life and addictions, that amphetamine or methamphetamine may be the answer. It works great for focus and as an anti depressant during the most difficult times of life. The goal is always get back to a balanced healthy routine without naturally occurring dopamine, exercise etc. You're a brave man to post these videos! There's a lot of ppl that suffer from severe ADHD that are self medicating with it, and who's lives you can see have a ton of potential if they were properly educated.

      We have a lot in Australia - it’s just that not all on PBS. The treatment of depression in relation to successful outcomes is about using what we have appropriately which does make a difference. Depression consists of domains and deconstructing these domains and using medication / psychotherapy/ lifestyle changes etc to target these is necessary for optimal outcomes

    Dr Rege off topic here sorry but wanted to ask you as I am trying to come off Tramadol 100mgs twice daily due to Australian Drs only being able to prescribe it once a month as of 2020 and you have to go to the GP for this in person. Is Cymbalta a good neuropathic pain killer? My Dr has put me on it at 30mgs going up to 60mgs in a month. Have you heard of people being able to come off Tramadol opioid type drugs by reducing and substituting it with Cymbalta for this sort of pain? Having to go to the Dr once a month every month for a 30 day supply of Tramadol is getting tiresome.

      Can’t speak specially in your case but yes Duloxetine is effectively used for pain and can help. However in the transition duloxetine plus tramadol poses a risk of serotonin syndrome hence has to be done properly. Also Duloxetine pain benefits occur above 60 mg. Ps not advice

      @@PsychiatrySimplified Thanks for the reply, my Dr has assessed the risk of serotonin syndrome and decided that the Duloxetine may benefit my neuropathic nerve pain and help me come off the opioids they kept me on for 31 years. What dose is considered effective to dull this kind of pain as I thought it was 60mgs?

    I have hypothyroidism, Hashimoto's, PCOS, ADHD and autism. I noticed I started having pmdd symptoms last month and then I started having them this month. Luckily I will be going in to visit my endocrinologist this week to go over my blood results and I started taking metformin for my PCOS but I noticed that since I started taking metformin some of my hypothyroidism symptoms came back. My boss said something that made me feel rejected and I ended up bawling my eyes out and then I had three more anxiety attacks after that when I went home and of course I'm looking for another job because hours are very low but I'm afraid what if I get like this and my other job in a couple weeks from now? I also see my psychiatrist this week so we would talk about many antidepressants / anxiety meds I can go on and maybe even have an anxiety Med that I can take only when needed and then have another one that I take daily on hand. Of course I will see what my endocrinologist says first to see what my blood test looks like. My hypothyroidism medication was working so well until a couple weeks after I started taking metformin.

      Thanks for sharing and hope you can address the issues you are having with your doctors. Metformin is known to reduce TSH levels and also prevents increase in thyroid volume.

      Yes that’s right. dopamine stimulates the production of thyrotropin-releasing hormone (TRH) and inhibits the production of thyroid-stimulating hormone (TSH). The HPT axis also regulates dopamine release.

    Dr. Rege, Thank you for another piece of jigsaw, in my understanding of the complex mechanisms of Dopamine, having SSRI induced EB and SD I was fully prepared to simply ask my GP for a dose of Dopamine, in my crude (at the time) understanding of depression I thought the simple release of Dopamine (the happy stuff) would be the answer. I now understand better, the real mechanisms and sequences of the processes involved, modulation is better than optimisation. I agree that the plethora of fixes and potions advertised for Dopamine is a dangerous thing, especially when there is no recognised scientific research to support them and no effective regulation.

    Thankyou doc .became a heavy drinker after people close died but coffee doesn't help either, tannins use up b vitimins .so i am down t one or two cups and ketvore . This is very important .
