The cost of cancer care is a topic at the center of a national discourse on fiscal responsibility and resource allocation. According to the Centers for Medicare and Medicaid Services, national health expenditures as a percentage of the U.S. gross domestic product (GDP) totaled 5% in 1965, but are expected to total 20% of GDP by the middle of this decade [1]. Although spending on cancer care comprises only 5% of the overall health care budget [2], these costs continue to rise at a pace more rapid than any other area of health care [3]. National cancer expenditures are projected to increase from $125 billion in 2010 to $173 billion in 2020 [4]. As an increasing number of expensive targeted therapies are adopted as standards of care, the average cost of treating common cancers is rising rapidly, with drugs accounting for approximately 40% of the overall cost of cancer care [1]. Another area of increasing costs is the use of diagnostic imaging. Significant annual increases in imaging have occurred across all major cancer types, and imaging costs have been rising at a faster rate than average total costs of care [5]. As a sequela of these rising expenditures, patients are shouldering an increasing proportion of the health care cost burden, often placing them under significant financial stress. Treatment-related costs have been shown to significantly increase financial burden among underinsured patients [6]. As we struggle to control rising national cancer expenditures, oncology providers are forced to examine practice patterns and their contributions to the overall health care cost burden. In 2010, Dr. Howard Brody presented a challenge to the leaders of all medical subspecialties to devise “top five” lists of costly treatments or diagnostics that lack the evidence base to support common use [7]. In response, the American Board of Internal Medicine has promoted the Choosing Wisely campaign, encouraging physicians to choose tests and treatments that are grounded in a solid evidence base. The American Society for Clinical Oncology, along with many other specialty societies, responded to the challenge and identified the top five areas for change in current oncology practice (Table 1) [8].

Thank you so very much for explains this. I’ve searched through so many videos etc on line looking for this info. Using Acellular Dermal Sheeting prevents a patient (who has had an immediate reconstruction with a saline implant) and later needs radiation it keeps it from contracture. My plastic surgeon explained that I could have an immediate reconstruction and he said he would put this mesh originally made from sheep guts around the saline implant and it would adhere to my tissue forming an inside bra f sorts. He said if I need radiation it would be a problem.

I was diagnosed with HPV tonsil cancer in June of 2022, at the age of 47. They said it was inoperable so the only course of action was 35 treatments of radiation and 7 treatments of chemotherapy. Before treatment started I was advised to have my remaining teeth pulled because it would take too long to get them in order and healthy. I also was fitted with a feeding tube and a chemo port. During treatment I took an anti nausea medication that caused a side effect of not allowing me to eat. My mind didn't allow my throat to swallow food. So that started me getting nutrition by feeding tube only. During treatment the only side effect that I knew of was skin burns on the side of my neck. After treatment in November of 2022, I still could not physically swallow food and I developed a severe headache on my right side and neck pain/stiffness. Doctors couldn't find a reason. 6 months after treatment and countless doctors appointments my wife came to the conclusion that I had an infection. So we went back to my ENT doctor with this thought and after testing was found to be true. So had a surgical hole placed in my neck to allow the infection to drain and was started on antibiotics. 2 months later, I had no improvement. So I found a cancer specialist doctor 3 hours away and he found that I developed and hole in the back of my throat from the radiation and in turn was causing my infection. I also had weakened blood vessels that ended up bleeding and me loosing needing 5 units of blood. I just had the vein to those vessels surgically blocked. The next step is a throat reconstruction surgery that will have a months long recovery time. I wish this on nobody and to keep the depression at bay has been the hardest thing to do and without my wife's support I honestly don't think I would have made it this far. Wish me luck please.

Informative and compassionate presentation. The standard of care for GBM has not changed for the last 10 to 15 years, regardless of the mutation. Sometimes I wonder if those who were "cured" had astrocytomas grade 2 or 3 but not GBM. It also seems to me that the younger you are and the earlier the tumor is found the better the prognosis. I believe that it is not possible to debulk the tumor completely because these are mutated astrocytes. The problem with cancer, and GBM which I think is the most terrible one, is that we do not have enough resources to really takle this.
