289 - Production - Based Pay For Support Staff
Dr. Peter Weinstein explores the feasibility of implementing a production-based compensation model for veterinary support staff with Cone of Shame host, Dr. Andy Roark. They touch on the potential benefits and drawbacks of this approach, discussing how it might improve job satisfaction, retention, and appreciation for technicians, assistants, and other support roles. Dr. Weinstein argues that while veterinarians benefit directly from production-based pay, the staff executing the bulk of the work often remain on fixed salaries, creating a disconnect in compensation fairness. The conversation also touches on broader topics such as profit-sharing and the overall drive to enhance workplace morale and productivity. They examine whether tying wages to service delivery could equitably distribute earnings and reflect individual contributions more accurately, potentially revolutionizing compensation structures in veterinary practices.

    Could you do one on vets and clients? I went through cohesive control from my vet. It meant my cat suffered horrendously. I liked and trusted those vet for over a decade. She knew I only trusted her most due to bad experience with another pet. But if made suggestions like my cat needs bloods done for checking thyroid and potassium levels to adjust meds appropriately, she would say no and make me feel bad if I did, despite cat needing it, it worked. Other things I suggested, my cat had nasal tumour and was on palliative care, she delayed obvious signs of pain and soreness but vet made out the steroids was enough and pain symptoms werent pain. She had obvious infection and needed antibiotics but I was afraid to go to another vet as didnt think she would be happy and may think I'm going behind her back, so I got it from a friend who happened to have some. She would have a blocked tear duct and face would swell like a balloon and burst. Vet made out that she wasnt in pain. Cat started crying and pacing, and I asked if it was severe pain from tumour spreading. She told me that cats dont cry and pace in pain. She told me it was sudden dementia, despite her memory being just fine. She said she isnt distressed nor is she aware. So I left her approx a total of 7 days in severe pain crying and pacing, having to have supplements syringed and also water. Turned out I was right and was severe pain from tumour invading back of eye where optic nerve is. It caused her eye to tilt. Over theast year, I wanted to put my cat out of her misery and asked if it was fair on her because she looked depressed and hid loads and breathing wasnt great. But because on the odd occasion my cat would pick up a toy (new behaviour that can indicate pain but I had no clue) that this was enough to be classed as quality of life. Then even right at the end when her face was swelling and bursting and It was obviously cruel. She convinced me somehow that I was wrong and guilted me into keeping her going. I knew this vet for many years from where I used to work at an animal sanctuary. She knew I had been bullied there and left, she knew I had past traumas and suffered anxieties and had trust issues and she understood why I only trusted her. I said to her after, had another vet called Emily spoken to me about the symptoms she would have put her to sleep. My vet responded by saying, ye she may have suggested putting her to sleep but you wouldnt have been happy without seeing me first. I said I wanted to put her to sleep ages ago and if Emily suspected it was tumour, I would have done it immediately. First attempt to put her to sleep and vet didnt want to, I said to her that I rather she went a little too soon than a little too late. My vet manipulated me to make it far far too late. I now suffer with PTSDA symptoms as a result of watching my cat crying desperately for help, trying to escape and trying to get comfort from me but I ignored her and shut her out of bedroom so I could sleep because I thought she wasnt suffering and just in a dementia episode that she is unaware of. She was very aware. It pains me so much. I wouldnt be here now if wasnt for family. It only passes the pain to others. So I'm forced to endure it. I'm losing my job and college place. I'm staying with family because I csnt stay at home where sure pain went on and suffering for a year more than she should have. Pain meds would have made a huge difference. Would love to know what this time of manipulation would be classed as. I know my vet didnt like to do things I suggested, looking back its plain as day. But she was so lovely in person, so understanding. I had several alarm bells telling me to change vets but fear of other vets from past experience and the fact I didnt want to hurt my vets feelings from knowing and liking her so much. Everything is so clear how wrong and obvious it was. The treatments and things I asked should have been protocol and I shouldnt have needed to ask. I did complain about her colleagues that hurt my other cat so not sure it revenge was in her mind. I stuck with her as didnt want my baby girl going through what her brother did but sticking with this vet meant she went through so much more suffering. Far more pain over a year long period, as opposed to him being under an hour with other vet. I hate myself so much for not seeing the obvious. Would love to know what you think.

    This helps with some situations in my practice this week. In terms of future podcasts, can you shed some light on the examroom etiquette for how to show the value and medical need of preventive care without coming across as a salesman to the client? Of course there is financial benefit for the practice for compliance but I would like the staff to stand on the soapbox for things they believe in above the fact that it's financially advantageous or because the manager is pushing them. I feel that it's hard to pull a script together with so much variability for each case, client and staff member. I would love it if my team was talking about nutrition and Nutriceuticals but I am struggling to get them to discuss fecal screening and a 4dx or dental disease in a grade 3 to 4 mouth.

    I let my Charlie Boo pass on to Paradise a little more than three years ago. Initially I questioned my decision because I could not allow myself to believe it was the right thing to do. I was in denial for months telling myself he did not want to go-but that was not true; Charlie was in pain for months but I could not see that. I blamed myself for missing the signs that he was in distress with a grade 5 CHF because I was ignorant about the condition until it was too late. What gives me, and my wife, comfort is that we gave him a great quality of life with lots of love every day. He gave us more joy than we could ask for. We gave him the dignity of passing on to eternity in my arms, at home surrounded by his family and comfortable. Thank you for your words of love and caring; and allowing me to share this moment. 😢😢

    Beware of Using Panoquel for Your Dogs! Our dog was sickened, and nearly died, to the tune of more than $4000, from Panoquel. It was used as instructed, and he had no pre-existing conditions or allergies. He lived through it, but the BIG Pharma company CEVA refuses to acknowledge any involvement or help with vet fees despite the drug nearly killing my dog. Stay away from the drug and CEVA.

      Hey would you be able to tell me more about it? Considering Panoquell for my dog. Was it administered during hospitalization?

    • @lukebrown625
      @lukebrown625 Před 7 dny

      @@meredithetalibon14 Our dog had a small amount of blood in his stool--a clinical sign of pancreatitis, so we took him to the vet. He was given a weight-appropriate dose of this drug. Directions are to administer it once per day for three days. After the second shot, he was fine. However, after he got the third shot on the following day per the protocol, and we were going home, he collapsed in my wife's arms and appeared dead. By then, the vet had closed, so we took him to an emergency clinic. There, they administered fluids and plasma over several days. He was later transferred to another facility with more specialized CT or MRI equipment to rule out other causes. The ultimate diagnosis was anaphylactic shock from the third shot. The treating vet had a hard time getting anyone at CEVA to reply to him, but they ultimately did. They refused to acknowledge that the drug caused the near-death and refused to reimburse care costs, despite being given vet records and having conducted no tests themselves. I have written to corporate officials of CEVA and the manufacturer, and posted of the danger on social media, all without response. During our vet's last call with CEVA, they acknowledged getting my communication but said, "They only communicate with vets." Interestingly, Dr. Heartsill (on the UTube interview) viewed my LinkedIn profile after I had written to her and other CEVA officials about the issue. Also, she explicitly advises not to wait until a dog's condition worsens before using her product--it should be used at the "first clinical sign" of pancreatitis. That's what we did. Panoquell nearly killed our dog and forced the expenditure to save him of approx. $4000. I hope this helps your decision and demonstrates that the manufacturer and CEVA don't care much about the danger Panoquell poses.

Instead I'm going to use my medical background to become a registered nurse. What pisses me off is that the only ones who are benefiting the veterinary field are managers and veterinarians. I had to put my 11 year old husky to sleep 2 days ago. She was strong up to the last day. We were able to do all her favorite things, walks, hikes, took her range and she got to mingle with all her favorite people. People were surprised she was running around for an 11year old dog. Had a great weekend out, brought her home, fed her..... Then in the middle of the night, she abruptly had a seizure. Her health nose dived off a cliff due to a cancer abdominal rupture on her adrenal glands/liver. We rushed to the ER and she couldn't even walk. She didn't want to eat. She seemed in agony. I had less than a day to brace for this. The vet said saving her was very slim and the best thing I could do was to ease her passing considering how fast she declined in less than day. It's gut wrenching. I think letting her go easy was the decision to make. Prolonging it would have made her suffer. I don't quite know how to accept her passing. It feels very empty and barren without her around

Worst of all is letting them suffer hoping that they will revive again. That is selfish you must decide quickly and get it over with. The longer you prolong the longer they are suffering. Food matters and the dog food companys have vets taught that dogs and cats should be fed dried garbage everyday of their lives and its sad! Not your fault but it will go a long way to help veterinarians be doctors and leave the diets to a nutritional expert! These are living breathing animals that are important to us and they need appropriate whole foods to thrive like ever mammal on the planet!

    One of the most difficult decisions to make. Our pets are our family members. My last one died mid exam at the vet. My vet thought someone had poisoned him. Have never had another dog since then, don’t think I ever will. 😢

    My gorgeous Patterdale terrier torn his cruciate ligament from chasing balls, I was so uneducated on not over throwing the ball! He loved chasing the ball but no more fun now 😢

    Thank you for your video. I'm struggling with this decision. My angel is my service animal and I love her more than anything on this earth. She is my soulmate. How can I cope? Can we talk? Please?

    Just put my little buddy down a couple days ago. His little legs were just giving out on him. What really hurts so bad is his spirit was still so strong. He still wanted to play fetch. But he couldn't hardly walk let alone run. He still was getting excited about getting his food, but he couldn't jump all the way from the kitchen to the living room like he used to. He wanted to bound up and down the steps or jump off the couch or bed. He just couldn't, or he would try and would fall down. When I had to help him just stand up in the morning to get out of bed, I called and made the appointment. My vet had tried a few different things. He had other health issues too and was already on several meds. His little body couldn't take much more. It was time but it still sucks and I'm having a very hard time with this one even though I've been through this several other times. But I know he had to be hurting and he's not hurting any more.
