Thanks for your beautiful video and explanation! 🙏 However, why do we have to do the Gibson approach and Ganz osteotomy, instead of a standard Anterolateral approach (Watson Jones) which also doesn't disrupt the posterior vessels? 🤔
Hi, yes you can safely visualise the femoral head with an anterior dislocation through a Watson-Jones approach but it is usually easier to achieve reduction and fixation of a femoral head fracture through the Gibson/Ganz osteotomy. You can also address the often associated labral injury or posterior wall fracture through the same approach.

11:06 is the screw out of the nail??
Yes it’s a blocker in the saggital plane to prevent the apex anterior or flexion deformity that you can get with subtrochanteric fractures. Sometimes you can put the nail down without locking and spot a deformity that can then be easily corrected with an appropriate placed blocking screw. As an alternative you can decide if there will be a predictive deformity and put them in advance, then you may wish to decide to create a balanced working length between blockers on either side of the fracture and on occasion even after all that you might realise you didn’t need them in the first place!
