Correction of TMD and Open Bite with External Plate and TADs - BTC #012
#BackToTheClinic
In this Back to the Clinic session, Dr Alikhani discusses the treatment of an adult patient with TMD and a moderate skeletal Class III and open bite malocclusion. The patient was referred to CTOR Ortho (www.ctor.clinic) by the ENT that was unable to address the patient’s pain. This patient skeletal problems and malocclusion were treated without surgery or dental extractions, using tools developed at CTOR (external plates) and a customized mechanotherapy approach. At the end of treatment not only was the malocclusion corrected and a functional Class I occlusion established, but all signs and symptoms of TMJ disfunction were also eliminated.

    Absolutely in love with your teaching method - very complex topic explained in great details . thanks a lot . Can you please share your views on V bend application in ANCHOR BEND principle in continuous arch mechanics : 1. in case of reinforcing posterior anchorage for anterior retraction 2. also in case of protraction of posterior teeth & reinforcing anterior anchorage with anchor bend specially the vertical forces generated with anchor bend in both situation & its clinical implications.

    Thank you Dr for detailed video. I have a doubt at 7 min 5 sec i understand the to as the distance increases & the angle of activation is maintained ; the force reduce - this is under the assumption to maintain the same moments - M1 = M2 ; this is my understanding. But in video you mentioned both force & moment changes - this is where the confusion - may be my understanding & assumption is wrong - how the moment can be different if the forces has to reduce when the distance increases & angle of activation is constant. so in the pic in video you mean to say : 1. M1 is equal to M2 or 2. M1 NOT equal to M2 or 3. M1 & M2 are separate diagram & are unrelated . Looking forward to your response . thanks in advance. Regards

    Hello Dr 🙏 I dont know if ull ever see this. But i thought id try. I was recently reading a paper that used a yinyang archwire for the correction of an occlusal cant. But i couldnt really understand the mechanics behind it. I came up with a theory, that the 2 sides of the wire since they extrude opp forces eq in magnitude and opposite in direction than they act as a couple, with the center of the maxilla bieng the center of resistance and center of rotation. But then what about the couple produced? Wouldnt its direction be opposite to the deactivated forces that i desire

    Wow. Amazing case. Too often physicians claim because adult jaws are done growing the only option a patient has is surgery. But then cases like this go to show otherwise. Cases like this highlight that even if surgery really is necessary to fully treat facial anatomy related health conditions, that there are going to be distinct benefits to using non-surgical treatment modalities to remodel the craniofacial complex prior to surgery. That the best outcomes aren't going to be purely surgical, but a combination of nonsurgical and surgical modalities. I wish more orthodontists paid as much attention.
