    The problem is that all the regulations are written by IMO in a warm well lighted, room in London and then implemented at onboard at sea under sometimes miserable conditions, by overworked, tired and under paid seaman's, on top of this with officers with little or no understanding of the proper rigging of the ladder. Note that the pilot poster is in English with the understanding that all seaman's understand the terminology used in the poster...when for most of them English is a second language...Why not have a poster in Tagalo? and see if that have an effect in the statistics?

    A report of unsafe pilot boarding arrangements could lead to further serious deficiencies being discovered by port state that otherwise might go undetected. These might be for example non payment of wages or even more serious seaworthiness issues that could even lead to loss of life if not rectified.

      It is exactly why all non-compliant arrangements must be reported to Port State control ASAP.

    Capt LeGoubin has hit the main topic in full, to train future masters you have to start from day one and not at the last minute. Even the good practice of the past and namley to have the C/O on bridge during mooring or unmooring maneuvers are today strongly discouraged by shore managements because otherwise they'll affect the hours of rest of the junior officers and then it clearly becomes a problem during oil major vettings and audits. It could be avoided with a good shore trainings with manned models but this too is avoided at all costs because it is too expensive. Did you notice the question about unruly crews? With the unruly crews there is also sabotage. It's a pity that today masters are reduced to mid-level operational managers with the operational top on shore and we know what is going to happen in case of problems.

    Many of the Old Timers did not share because they saw the young, better educated and often better trained recruits would soon 'take their jobs'. They guarded their knowledge jealously and were reluctant to write it down for others since it would hasten their 'redundancy.' They were, of course, not wrong from a purely personal perspective. For example, when digitalisation first appeared many were unable to adapt. Now ships have their bookcases full of manuals on how to run everything on board but still make expensive yet simple mistakes that the old timers would have scoffed at.

    Thanks! A very informative and inspiring talk. Being a sailor for 30 years it is criminal the way seafarers have to struggle with terrible garage made energy vampire software. Even larger maritime software companies piggyback on Lotus 1-2-3 in 2020!

    Well, 'Over-reliance' on checklists is forced upon us. We need secretaries! Normally, there is so much paperwork, that the poor cadets and JOs end up swamped in it, rather than spending the hundreds of hours watching the master at work, which is necessary to progress. Training and procedures are very different from education and paperwork. Time on board is limited, and for deck officers, i believe it should be immersed in action and observation & problem solving. Not box ticking.
