I have had a patient with post cardiac arrest shock receive eCPR/ECMO cannulation in an emergency department who had an excellent outcome that I am fairly sure may not have turned out as well without this resource. Excellent lecture.
Scott, Thank you again for everything you do. This video was very helpful review and I have a much better understanding of these concepts. The research you have put into one of the most important factors affecting our specialty is amazing and I cannot thank you enough for your devotion to this issue.

I personally not a fan of tPA - I personally haven't seen any benefit yet. We are not sure if its some thing going to change my practice. Its all about legal litigation, follow hospital policies etc etc. Great to hear that I am not the only one who is confused.

One thing I personally am missing from this is eye protection. It's the one thing which is most frequently forgotten. Otherwise, this package is awesome!

You’ve used your hiatus to build a well thought out intubation kit-but please don’t undertake another project if it will interfere with regular uploads 😉
But stand on your dominant side not at the head side ,you can control this procedure better ,you can anchor your hand on pt chest . Scalpel, finger bougie and tube Now no more hooks or tracheal dialator. Finger is a good dialator and blood is good lubricant and if one can feel posterior tracheal rings then straight go for tube as it's a peri arrest situation

8:00 FINALLY someone else who does it this way. I learned this method from an OR nurse and I've gotten many weird looks for doing it this way. But if you touch the gloves with your bare hands, upon sliding them up your hands, they will almost always roll up against your skin and become unsterile.
Why not insert a tube exchanger or bougie once you have access to the trachea? It seems like this will make placing the tube much easier. For that matter, why not access the trachea as you would if you were placing central line? Advance needle under negative pressure aspiration and then wait for air to come back the same way you would wait for blood return when placing central line. Cannulate with IV, thread wire through cannula, then cut where the wire is down to trachea. THEN put tube exchanger over wire, THEN put tube or trach over tube exchanger. It seems like this would be much easier for someone who has done very few of these... which will be almost everyone watching this video. It leverages skills that most providers are already likely to have from placing lines. Would be interested in your feedback on this approach.

this is an in-hospital bag, so VL should be mounted on a cart. You can cram in a portable if you are going to a floor
Epic bag! We use this on my PHEM team! It has streamlined our airway management and ideal for aiding in prehospital RSI’s!
Got to perform several of these during my MICU rotation. Glad I watched this & not have looked like an amateur!

Would you be willing to share your hypothermia protocol with me? I am the ED director in a small, rural hospital that has had to ship a large number of patients in the past. Now that we have been taking care of so many sick COVID patients I believe the staff in both ER and our ICU are ready to have a hypothermia policy.
Is it still recommended?
Protocols have changed slightly, but targeted temperature management (TTM) is still recommended for post-arrest patients once ROSC is achieved.
