so not only do you pay your insurance company a loyal amount each year/month, usually only affordable via companies as it's too expensive for a single household (leading to being pressured to stay working at your job), these insurance companies will then fight with tooth and nails as to why they should not cover you with the insurance YOU ALREADY PAYED FOR to the point you are almost always likely to pay large portions of the medical bills yourself, you then also can't even be helped by any hospital or doctor and need to find one's that are "in network" (don't even want to think what would happen if you are transported via emergency vehicles to the nearest by hospital with no guarantee they have doctors that are "in network" for your insurance). The first thing you learn as a non-american is that their healthcare is ridiculously expensive but that isn't even the biggest problem, it's a scam. Everything is set against you. And in more ways than one. You could lose your job when in the hospital even if you can proof you couldn't work. You then get a 2 week notice to move out of your rented space because you are now unemployed. You can't leave the hospital so your landlord can legally move all your possessions to the garbage that you then have to pay for. You leave the hospital homeless, unemployed and in crippling debt. And then americans wonder why so many of you are homeless. I'm speechless.

In the United States, annual enrollment (also known as open enrollment or open season) is a period of time, usually but not always occurring once per year, when employees of companies and organizations, including the government, may make changes to their elected employee benefit options, such as health insurance. The term also applies to the annual period during which individuals may buy individual health insurance plans through the online, state-based health insurance exchanges established by the Patient Protection and Affordable Care Act. Prior to January 1, 2014 insurers offering individual medical coverage typically allowed new members passing underwriting to enroll at any time throughout the year. Annual enrollment is also prominent in Medicare, where almost 50 million enrollees can choose to stay in original Medicare, or join or change plans within the Medicare Advantage and Medicare Part D Prescription Drug programs for the coming calendar year. Individuals usually can make changes to, or sign up for, their health insurance or fringe benefits only once per year during the annual enrollment period or when they have experienced a specific qualifying event. Open enrollment periods are used in insurance markets to limit adverse selection risks resulting when enrollees can switch plans at will. During this time period, an employer will typically communicate to all eligible employees what options they have for their benefit program. Often, the vendors or insurance providers will be present to explain the details of their products. This can be done either with group presentations, "benefit fairs", or one-on-one meetings with each employee. As travel expenses continue to rise, many vendors and insurance providers have turned to using independent "contract enrollers" to do the communication on their behalf. Some companies and organizations distinguish between an active enrollment benefits election period, where employees must re-review or confirm their benefits selections for the coming year, and a passive enrollment benefits election period, where employees are automatically renewed with their existing benefits selections from the current year if no action is taken. Open season is a prominent feature of the Federal Employees Health Benefits Program, during which some three million federal civilian employees and retirees may choose among several dozen health insurance plans for the coming year. Open season is scheduled in the fall each year, and plan enrollment decisions take effect in the following calendar year.

One year after my employer switched from a PPO (I was in excellent health) to an HMO, I was totally disabled because a specialist my primary care doctor sent me to did something he shouldn't have done, and for the next year as I got sicker and sicker, my primary care doctor said nothing was wrong. I finally got into the VA Healthcare system and found out the primary care doctor had been lying to me the whole time. When we tried to sue, we found out you can't sue an HMO or its doctors because you signed an agreement to let some bean counter at an insurance desk practice medicine without a license. In 1997 Congress had to pass a law barring Medicare HMOs and their doctors from lying to patients about their condition. This only applied to Medicare patients, as Medicare falls under the Department of Health and Human Services (HHS), but they were and are allowed to lie to everyone else. I was lied to before 1997 and was not in Medicare. For your own sake, Google "HMO's & Gag Rules". If they weren't doing it Congress would not have had to pass a law. Humana's HMO took my life. If you value yours, stay away from HMOs. Now for the past 3 months I've been suffering with a pinched nerve in my neck (C6-C7), but my Medicare HMO (AvMed) won't approve my pain specialist to do a nerve block to diagnose the correct vertebra for an RF Oblation to relieve this excruciating pain. This is what you're in for if you sign up with an HMO. When they put you through this kind of agony, all because some bean counter won’t approve a procedure that your doctors says is necessary, you will wish for death, not just for yourself but for the insurance company making you suffer, and for the Congress that allows this to happen.😖🤕☠

For most people, it comes out of their paycheck. But, if your employer doesn't offer healthcare benefits, you might have to purchase health insurance on the Marketplace. In that case, you'd pay it like any other bill. Marketplace is waaay more expensive usually.
