いい情報ありがとうございます。 検査結果の日本フォーマットには氏名、パスポート番号、生年月日は印字されて いますか。 追記で、自分で記入するのでしょうか。
日本のフォーマットには、氏名、生年月日を含むその他結果について、検査所の方が手書き記入済みの書類がpdfで送付されます。パスポート番号は日本フォーマットには不要なので記入されていないです。 また、検査所オリジナルフォーマットには、氏名、パスポート番号、生年月日が印字されたものが送付されてきます。
