    Thanks for watching I am one of the patients who had spine surgery due to herinated disc. Now I am one year two months old. I am ok. My surgeon told me every thing concerning the surgery and I was convinced. Thanks be to God for giving these people wisdom

    What about pain around c2 or 3 (with a crunchy thing) that causes massive headaches. It starts at the base of the skull and radiates up to the temples. And switches sides. Also for me I have pain that goes down to my scapula. From my levators. I can't get those in balance no matter what I do.

    Respect to your channel Dr. , For at least 15 years Dr's in the US said NOTHING could be done for my DDD. Forced out of work many times, pain management, injections, spinal cord stimulators etc etc.. I have seen every top spine surgeon in South Florida (at least 20) NONE offered help of any sort. I finally went to Germany 2 Level Lp-ESP at L4/5 L5/S1 and the relief was instant! Millions of dollars wasted, and nearly 20 years of my life destroyed for a 40k procedure, it was that simple! AND guess why I needed a 2 level surgery, BECAUSE the Americans are behind in there thinking, ASD had already moved up to the next level. My surgeon in Germany has placed over 5k ADR without an access surgeon, they are dumbfounded at the Americans!! SAD!!!

      You are correct: Spine surgeons in the U.S. generally follow very similar clinical/ surgical guidelines (for a variety of reasons related to insurance/ payments/ medicolegal-considerations/ FDA-regulations/ accepted standard-of-care criteria etc). Therefore some treatments which are commonly available in Europe are not as easily available in the U.S. So your observation is accurate.     Why not put a cage in addition to posterilateral fusion ???

      If you look at the literature, the pros/cons of a fusion with cage balance out with the pros/cons of a posterolateral fusion without cage: Fusion with a cage requires a unilateral facetectomy, so automatically the concurrent posterolateral fusion has to rely on the contralateral side only (with the remaining facet). Also, the incidence of cage-subsidence is not insignificant (especially with osteoporotic bone). Additionally, sometimes the cage can back-out and impinge against the nerve root or the cauda-equina: In that case removing the cage can be very dangerous (since the dura is scarred down very tightly). For these reasons the surgeon has to balance the pros/cons of using the cage for each patient. Your question was excellent; maybe I will make a video about it 🤔

    I am thinking about surgery on my lower back.a brief history, I have had all injections possible i think. im taking perocet 10 mg. being doing exercises for back and walking every day. Had a stimulator put last year and all of this just don't work like it use to. this has been going on for about 10 years. He ask me about surgery several months ago but declined but know im thinking about the surgery. do u have any recommendations at this time? By the way im 76,

I think the best way to determine if you need surgery is to get a consultation with a spine surgeon; if you are not satisfied with the recommendation of the surgeon maybe you should get a second opinion. At age 76 you have to balance the pros/cons & risks/benefits of surgery vs. no-surgery very carefully. The risks of surgery rise as you near 80yrs of age. Wishing you all the best 👍

      Obviously I cannot diagnose your condition, but you may want to see an orthopedic surgeon for evaluation to rule out a cubital-tunnel syndrome.

      Getting this problem evaluated/ treated by a chiropractor can be quite helpful. The chiropractor can determine if you may need a referral to a spine-specialist if the problem doesn't improve with conservative care.

    Hi! I was dx with DDD in 2000 at the age of 35. In 2010, I had C5-6 spinal fusion. Currently, I have multiple issues in my lumbar spine. Started treatment for this issue in April. MRI, PT, Pain management, neurosurgery consult, multiple new meds for nerve pain. Had one ESI without any relief. There are plans for a second ESI and a nerve block. My pain is at the L3 nerve root and has disabling. What are your thoughts on a course of oral steroids vs. a second ESI? I have read some research on this topic. My thoughts were systemic vs. failed localized. Thank you in advance!!

      I say no to oral steroids. You need movement, not drugs or procedures.

      Epidural injections are very safe for the vast majority of patients. Obviously there are exceptions, but they are rare. This is something that you can discuss with your pain-management physician. Good luck

      Sorry to hear about the very trying situation which you find yourself in. Your question is difficult to answer for me because only a surgeon who has examined your father (and also reviewed the history and imaging-studies) can make such a determination. I hope that your father has been evaluated by at least one spine surgeon.     Non smoker/drinker/druggy. Had a re-fusion L4-L5 after finding out 30-years later that the first doc didn't do a good job. New Doc did a "360" S1 thru L5 then up to thru L2. Also both SI joints got screwed together. Post op 2-months, I'm doing good feeling really great no pain in legs/back. After simply pushing on my truck just a little to get it rolling into the garage for repair (crank sensor)...BAM...now I'm bent over with spasms at my belt line right side. A week has gone by and it won't stop hurting spasm sudden shock of pain for no reason or movement. Saw my Doc he doesn't want to guess. Just had CT scan today....hope nothing broke or came loose. I was doing so well with high hopes....

If you are having tingling in the arms, I suggest that you get an in-person evaluation by a spine-surgeon. Sometimes tingling in the arms can be a sign of spinal-cord cord compression, which can be a serious problem. Before trying physical therapy, I suggest an evaluation by a spine-surgeon.     I had a laminectomy last year, felt great for 5 months and then the pain came back Now they want to do a fusion. At first they said alif at the L4-5 level where they didlaminectomy. Since i got an injection at L4-S1 and avtually felt relief for a week. So now if they go in i want both levels taken care of so i dont become a surgwry customer.

    Hello doctor, I had been having back ache issues for a long period, but since January the pain was unbearable and I consulted an orthopedic doctor and he suggested some pain killer and nerve relaxation medication. It used to work sometimes and sometimes not. In march i took a cordalblock injection, it gave me relif,but on may 19th the pain suddenly reappeared and it was totaly bad. I got pain killer through IV but not much help. I unbearable went a spinal fusion surgery on L3,L4,L5 and S1. There is no pain sincethen but the strength on the affected leg is very less and I am using a walker for my needs. Started physiotherapy since discharge and now zi am able to walk without the help of a waker but my hip falls when walking and I am not 100% as I cant walk like normal people. Can you suggest what has gone bad in my condition. Sujit

      Hello Sujit, Thank you for reaching out. Obviously I cannot comment on your specific condition (since the particulars/ details are not available to me), but I can tell you that it is not unusual to have some transient weakness after surgery (if the nerves are severely compressed before surgery). For most patients the weakness gets better over time (may take over a year) with physical therapy. Hang in there, be patient, stay positive. Recovery after spine surgery takes a long time.     I had to laminectomy L5 S1 2012, 2013. After surgery six weeks later the pain was back scar tissue grew. I couldn’t feel my foot. 6 months later surgeons operated & cleaned scar tissue. Six weeks later scar tissue back. Today new MRI scar tissue grew 3x’s bigger. Is there any new procedures to help me? I’m in horrible pain now. Foot is on fire, cannot wear sock, pain is unbearable? Injections help 20%

      Thank you for sharing. Since I am not familiar with your case, I cannot recommend specific treatments. But have you discussed the possibility of trying a Spinal-Cord-Stimulator with a pain-management doctor? That could be one option.

    I have c5 trapped nerve, very painful, Ive had physio therapy for 3 years with no change. Any idea of releasing this nerve when its inflamed?

      Thank you for your comment, I understand your concern, it seems quite valid. I hear this quite frequently from my patients.I think I need to make a video about this exact topic, which may turn out to be quite interesting. The fact of the matter is that medical-device manufacturers have spent billions developing and marketing ADRs, but (to their surprise) with mixed results. If ADRs had turned out to be wildly successful (and therefore superior to the traditional fusion), surgeons would have stopped doing fusions. Also, as an insider, I know that ADRs are much easier (and faster to perform) compared to a fusion. Despite these realities, disc replacement has still not been adopted by the majority of (very experienced) spine surgeons. How could that possibly be? I think that unfortunately the word "fusion" has developed a negative connotation, and in my opinion a better term would be "stabilization." Sometimes it is necessary (and unavoidable) to stabilize the spine.     I had L4-5 laminectomy/ Discectomy and had more pain after. after PT for six months and injections I got second opinion. They said my spine was unstable and nerve crushed so did L4-5 fusion. Still had a ton of pain for the next 2 years. I had mri n ct scans. The surgeon said everything looks great. He said maybe it's your SI joints. He said it's common after spine surgery. So I did PT for two more years and more injections. After that, I ended up with 6 ifuse rods across my SI joints. Now I'm in more pain. This neurosurgeon that did the Ifuse did an MRI and he noticed the screws in my nerve canal. He said this didn't look right. So now I went to a trauma surgeon to help me figure it out He said on your L4-5 fusion you have two screws sitting inside your nerve canal. You need to get those out and he doesn't do spine surgery. I went back to the surgeon who put them in and they said we take full responsibility. I was fused so they took the entire cage out and left the plate. Went for my follow up with the surgeon and they changed it to the PA. They said that he will not see me. When I saw the PA, he was in the office and turned away and wouldn't even talk to me on my husband. The pa said he won't see you. Now I'm in the process of getting the ifuse rods out at Mayo Clinic in Rochester MN. All my surgeries were done in the Boston area and I thought we had great surgeons. I am terrified now. I wish I had someone like you in the beginning. This has been life altering. I've got four children and two little granddaughters and I hardly ever see them and it breaks my heart. my husband and I have not gone on vacation for the last five years I am basically in bed on an ice machine, majority of the time I try to get out and walk, but it's awful. Most of this started when I had an RFA,m on l4-5 anna S 1 which put me in the hospital for five days. I also found out I am allergic to titanium. I have severe burning where the rods are. It's pure red across si joints which is reason for ice machine. I love your honesty, telling people not to jump into surgery! We need more people like you for sure this world
