IV Conscious Sedation

IV Conscious Sedation Conscious sedation is performed intravenously, when medically necessary, to make our patients safer for a more effective and more comfortable experience during a procedure to try to help with chronic pain. Radiofrequency Ablation (RFA) and Medial Branch Block (MBB)

This procedure temporarily “turns off” the nerve's ability to transmit pain signals from your spine to your brain. This is done by destroying a portion of the nerve with high frequency radio waves. These nerves do not control any muscles or sensation in your arms or legs. With these nerves “turned off”, you aren’t able to feel as much pain that begins in your spinal nerves or joints. With time,...

    I gave up on RF ablations in and out like a revolving door every five months and went to a different surgeon for severing the pain nerves all together to seven cervical facets. That was in December of 2022. It’s August 2024 and have no regrets. I see no reason to keep exposing a patient to surgeries when a permanent surgical option exists.

    • @Gonzo-z6n
      @Gonzo-z6n Před 13 dny

      Are you talking about endoscopic rhizotomy?

    • @jerryrobinson7856
      @jerryrobinson7856 Před 8 dny

      @@Gonzo-z6n I had an older surgeon in California who used an old laser technique he had perfected. It’s permanent and I had it at seven cervical levels. He is likely retired. My own views on surgeons is they should have ‘some’ grey hairs. I will use a neurosurgeon hands down over an orthopedic surgeon, with the exception being a specialist orthopedic. For resources, there is Dr. Daniel Lieberman a neurosurgeon in Phoenix and I had a write in your question on his CZcams channel about a year ago about this. He indicated there was also a version of this to cut the nerve. There is also Dr. Ara Deukmajian in the Orlando Florida area who uses a very strong energy method. When I contacted that office about a year ago they indicated no patient had needed a second round. Deuk Spine it’s called has a CZcams with live surgeries. I highly suggest people go watch one of the exact procedure you are looking to have. It builds confidence because knowledge is how you make better decisions. The problem with mainstream RF Ablation (commonly practiced) is the nerve is burnt only to heal back in 4 to 8 months. In some cases the nerve is damaged and causes even more pain. I wanted a permanent solution. Let’s be clear, this did cost me plenty out of pocket in addition to insurance as the procedure is not recognized by insurers. The US healthcare is messed up, but it’s the cards on the table we half to deal with. I have flown for that laser cut nerve surgery and two other artificial disc surgeries in Barcelona to a specialist neurosurgeon who at the time had performed 2700 disc implants of which he had 550 lumbar implants with only two revisions. Germany is where the artificial disc was invented in the mid 1970’s and Europe can be well ahead of the US. The US is stuck with being on average five to eight years behind. There are exceptions by a specialist that limits their practice to just a couple procedures, like what I had performed. With that said, I would not trust any surgeon using laser. I require a minimum of 1000 operations of that nature and a very low revision rate. As to travel, it’s a minor inconvenience and to be honest, we would be there for a week or ten days and find touristy things to do.

    I had on both trials of over 85%, relief which is passable. Medicare has rejected me for no reason. So, II I can't get the Lumbar Radiofrequency Ablation only a few days before procedure. I met all the requirements. I told Medicare that my pain management doctor has been doing this for years., I double checked the paperwork, They were giving me the run ,around, Finally got another department who can go over their heads the person is a Level 3. I should hear this morning. They will expedite the order, so it can be done faster. Today was the procedure. I waited over 6 months. Reading the letter I got a immediately 10++++++ for pain. I then got very depressed like I wanted to commit suicide or kill someone which I had no intentions.. I went to Emergency, I can walk little in a walker, so I used a wheelchair. They gave me a morphine shot, which bought the pain down to a 6 and lessen my depression. After 2 days pain and depression is coming back. Medicare knows. I have a home care doctor coming today at 3PM. I might be going to the hospital for 3 days then to Rehab until everyone gets their act together. I can't trust being home, there is only my wife. Last year, I had 5 times I could not get up went to 4 different hospitals. This should have been treated as a em3ergncy, while waiting for 20 surgeons told me I needed to go to the Cleveland Clinic main campus. I have scoliosis that is a right curve that is lateral which rotates like a snake. L3-L5 are pinched and can't' operate. Eventually I will be bed ridden...

    Bupenorphine is not good. I was on it for years. Truly robs you of your happiness and soul. And it really sucks to come off of. Takes 30 days about to detox plus the after effects linger for months. Some people need pain management I get it but Bupenorphine is not the answer. It’s just a cash grab at the end of the day and it’s literally just like putting heroin and morphine into your body the first few days but then you will feel nothing but emptiness and sadness as long as Bupenorphine is latched onto your receptors.

The next day, I did feel some relief and today is Friday and I still feel some relief and today was my follow up with the doctor. Was told that if I didn't feel relief within 4 hours on the day of the first MBB, then probably not going to work for me. Was told that it would be unusual for the numbing agent to last this long. In the video, the doctor says the MBB could last hours, days, weeks. I'm confused now.

I didn't have any pain or sweating while standing in one place waiting. But next day was having a little pain, it like comes and goes. It may take a little longer to get the continued relief. So I am hoping for longer relief in a few more days.

    It's an overrated drug. The side effects of this so called Medication Assisted Treatment are not highlighted enough and really cause further complications. The only advice you're given is drink more fluids eat more fibre which of course does not help or deal with the side effects. Constipation, Severe Headaches, Insomnia and Muscle Aches just to name the main side effects but of course when they are ongoing they lead to other health issues but seeing as it's a relatively new medication the side effects are not highlighted but do the research and you'll see what I mean. You are advised if they persist or get worse then inform you're doctor who of course because it's new tells you to drink, eat fibre, etc etc. Give it a couple of years. These side effects will lead to the domino effect where they lead to complications. It will happen.

    Mmm I’ve tried this and had a very bad reaction to this med. I have pain caused by Post Herpetic Neuralgia (from shingles), and tried the patch, slow release, and at the 15 and 20 mark, (after being commenced at 5 micrograms per hour) I had a bad reaction: brain fog, itching, hives. No respiratory problems though and when I visited the doctor in person my blood pressure was perfect. In retrospect, as I had no idea that my reaction was my meds in the first instance and I was too busy throwing up to go to my doctor I did not realise my patch at 15 was the problem. So at the next dose (20) i had complete brain fog exacerbated by a melt down over a temporary inability to find a lost document. Over the two/ three week duration I lost 21 pounds and have missing days in writing my journal. I did have only some relief of pain. My next choice is oxycodone which I hesitate to take.

    My PCP just suggested this, but I have to see my pain management Doctor next week, and see if she's on board. I can't keep going like this. To prevent having to go up in pain meds, I've been supplementing with Kratom which is its own beast. I'm really hopeful I can do this.

It felt like i dosed off for a few minutes and woke up and it was all done!!! No pain either!!! Great and effective way of surgery 100/10 guys, don’t worry at all 😁😁😁

    question for Dr Suderman: in ASC surgery, some physicians use a disc release, which may cause an accelerated degeneration. Can the disc regenerate itself at a young age (20)? Is there anything to do (therapy., sports) to ease the pain of degeneration?

      Non generic medication will sometimes be ineffective. If you can switch to brand name the quantity may help I use Subaxone film and always request DAW. I have found a considerable difference in the efficacy and when you are in pain that's huge. Good luck to you.

      I actually am a patient of Dr Javery, in fact I'm going in today in about 4 hrs for my injections. I've been receiving some form.of injections here since the end of 2019. I don't know if I talk during the conscious sedation very frequently (if I remember I may try to ask today if I spoke at all during the procedure.) There were a few times where the sedation wasn't having as much of an effect as it had the first handful of times, and I remember a great deal of discomfort (not necessary "pain pain" but it definitely was unpleasant) and during those times I do recall expressing said discomfort, kinda tearing up/crying a touch etc. They've since adjusted the level of my sedation/amount of medication and since then I don't have any recollection of speaking. Though I'd be curious if I do speak and if so what I say lol. I've had remarkable results as well since beginning the injections. As with the gentleman in the video it's changed my life. I was ablento begin working, and enjoying more pain free days.

      I quit buprenorphine 51 days ago after a solid 15-16 year ride on it. I tapered down to about 1.5mg a day before jumping off. First 10 days or so were rough followed by another couple of weeks of poor sleep/energy levels but improving daily. Getting there gradually and happy to be free
